The form requires responding firms to confirm eligibility requirements including agreement to the State/Professional and Technical Services Contract, provision of a Certificate of Insurance, submission of a signed Affidavit of Non-collusion, completion of a Workforce Certificate if applicable, and disclosure of any foreign outsourcing involvement. The proposal must be signed in ink by an authorized officer or member of the responding firm, and firms must provide their State Vendor Number, Minnesota Tax ID Number, Federal Tax ID Number, and relevant professional registration numbers. The document does not specify pricing terms, set-asides, contract duration, or renewal options, as this form focuses exclusively on qualifications and eligibility documentation for the selection process.
