1.0 GENERAL INFORMATION
1.1 PURPOSE OF REQUEST FOR PROPOSAL (RFP)
The South Dakota Department of Health, Office of Child and Family Services, Home Visiting program is seeking proposals from partner organizations to provide evidence-based, voluntary home visiting services using the Parents as Teachers (PAT)® model for South Dakota families with children from birth to kindergarten entry. Services should be provided in selected, at-risk communities identified in the South Dakota needs assessment. Home visiting services are intended to be implemented at the community level as part of an integrated and coordinated system of maternal health and early childhood services.
Programs will collect and report program data and implement a continuous quality improvement plan to demonstrate progress towards intended goals.
Background: The Maternal, Infant, and Early Childhood Home Visiting (MIECHV) Program helps pregnant people and parents with young children improve health and well-being for themselves and their families. Families choose to participate in home visiting programs, and partner with health, social service, and child development professionals to set and achieve goals that improve their health and well-being.
Expected outcomes include improvements in maternal and child health, reduced child abuse and neglect, reduced incidence of crime and domestic violence, improved family socio‐economic status, improved child development outcomes and school readiness, and improved coordination of referrals to community resources and supports. PAT meets U.S. Department of Health and Human Services (HHS) criteria as an evidence-based early childhood home visiting service delivery model eligible to be implemented with MIECHV funding.
Eligible Service Delivery Areas:
Identified geographic areas (South Dakota Counties) are eligible for MIECHV funding based on the MIECHV definition of at-risk community and the 2020 South Dakota Home Visiting Needs Assessment.
Since several of the identified, at-risk counties have existing home visiting programs funded by MIECHV or tribal MIECHV, only the following at-risk counties are eligible for service delivery proposal under this
RFP.
Funded services are limited to the following counties: Beadle County, Bennett County, Brule County, Charles Mix County, Corson County, Dewey County, Hughes County, Jackson County, Minnehaha County, Mellette County, Walworth County, and Ziebach County.

3.0 SCOPE OF WORK
1. Provide home visiting services to eligible families using the Parents as Teachers (PAT)® evidence-based home visiting model with fidelity and adhere to model requirements. Follow model reporting, quality standards, and guidelines to obtain and maintain standing as an affiliate site with the model developer. Provide home visits on a voluntary basis to clients on a schedule consistent with model requirements. Conduct screenings and assessments with families as outlined by model requirements.
2. Identify and propose a qualified staffing plan that provides the infrastructure to provide home visiting services according to model requirements. Ensure that all staff (home visitor(s), supervisor(s), etc.) complete required model-specific training. Maintain high-quality supervision in alignment with model requirements. Identify the projected number of families that will be served and demonstrate an ability to reach projected target caseload and maintain at least 80 percent of caseload within two years of funding. There are many factors that can impact caseload size, such as the size of the service delivery area, travel, visit frequency, or other factors. Consider specific factors in the proposed service delivery area that could impact caseload size. It is important that your target caseload is attainable, sustainable, and utilizes funds efficiently. All home visitors must be at least .25 FTE for the caseload to be recognized and counted per MIECHV guidelines. Complete and submit the staffing and caseload plan template found in Appendix A.
3. Identify and propose a service delivery area in accordance with the guidance outlined in section 1.0 Eligible
Service Delivery Area. Cleary identify which SD county or counties you propose to provide home visiting services
in. Home visiting services are limited to households with addresses located in the identified, approved, at-risk South Dakota counties at the time of enrollment.
4. Identify proposed participant eligibility criteria and give priority in providing services to families that are MIECHV priority populations. Identify any subpopulations of eligible families that you intend to prioritize. MIECHV priority populations are as follows (not listed in order of priority):
Low-income eligible families;
Eligible families with pregnant women who have not attained age 21;
Eligible families that have a history of child abuse or neglect or have had interactions with child welfare services;
Eligible families that have a history of substance abuse or need substance abuse treatment;
Eligible families that have users of tobacco products in the home;
Eligible families that are or have children with low student achievement;
Eligible families with children with developmental delays or disabilities; and Eligible families that include individuals who are serving or formerly served in the Armed Forces.
5. Complete an assessment of community needs, readiness, and capacity for home visiting services.
6. Develop, implement, and maintain a formal system for obtaining and documenting referrals to the home visiting program. Develop policies and procedures to screen and enroll eligible families in the home visiting program that best meets their needs to avoid dual enrollment with other State funded home visiting programs. Coordination must occur with other state and MIECHV federally funded home visiting programs in the service delivery area.
7. Ensure at least 60% of home visits occur in person, as demonstrated by annual reporting data. Additionally, per
MIECHV requirements, each family should have at least one in-person visit every 12 months.
8. Comply with data tracking and reporting requirements as determined by the State in conjunction with the federal MIECHV Program as outlined in the performance measurement plan. MIECHV performance reporting details can be found at: https://mchb.hrsa.gov/programs-impact/programs/home-visiting/miechv-data-continuous-quality-improvement. Parents as Teachers National Center supports VisitTracker Web as their data system of choice and access to VisitTracker Web is included as part of the PAT affiliate fees. Use of VisitTracker Web is required to ensure streamlined data collection and reporting.
9. A proposed client consent form meeting organizational, State, and model requirements shall be submitted to the State prior to service delivery. Consent forms must note that client data will be shared with the State. Obtain consent from all clients that enroll in home visiting services funded by the State. Home visiting services must be provided on a voluntary basis.
10. Participate in required trainings and continuous quality improvement (CQI) efforts provided by the State.
11. Provide monthly progress reports to the State, using the templates and/or data systems provided.
12. Allow records to remain property of the State. This includes following HIPAA policy and procedures relating to client records, to include maintaining confidentiality and keeping client records in locked files and protecting records from loss or use by unauthorized persons.
13. Adhere to Department of Health marketing requirements. Written approval is required for use of the Department of Health logo. Co-branding of agency logo and program information is encouraged.
14. Establish or participate on an existing local advisory council for the home visiting program.
