About this file
This is a Proposer Information and Certification Sheet for RFP S-10700-0005442, issued by the State of Oregon's Department of Administrative Services (DAS) Statewide Procurement Services (SPS), regarding Statewide Environmental Analytical Laboratory Services. The document serves as the certification and attestation form that prospective proposers must complete and submit with their proposals. Proposals submitted in response to this RFP constitute firm offers valid for 180 days following the closing date. Upon award of the Master Services Agreement, selected proposers agree to perform the scope of work and meet performance standards as set forth in the final negotiated agreement. The specific details regarding response deadlines, site visits, bidders' conferences, award dates, and contract terms are not included in this certification sheet but are referenced as being contained in the primary RFP documents and Attachment A.
The certification sheet requires proposers to attest to compliance with multiple statutory and regulatory requirements, including Oregon tax law compliance, non-discrimination in employment and subcontracting practices, prevention of sexual harassment and assault (required for contracts anticipated to exceed $150,000), wage equity under ORS 652.220, and exclusion from federal sanctions lists maintained by OFAC and SAM. Proposers must also certify the absence of conflicts of interest with the State and affirm that all proposal contents are truthful and accurate and prepared without collusion or fraud. False or fraudulent statements may constitute claims under the Oregon False Claims Act (ORS 180.750-180.785) and subject proposers to associated liabilities and penalties. No pricing information, set-asides, incumbent contractor details, or funding sources are specified in this certification document.
