M00264-23-R-0002 Attachment 3 – ASSESSOR PAST PERFORMANCE QUESTIONNAIRE
Instructions:
· Sections I through III are to be completed by the Offeror/Major Subcontractor and provided to the Assessor (Government personnel who had experience with the Offeror/Major Subcontractor on a relevant contract referenced on Attachment 4). The Offeror must submit with its proposal (by the closing date of the Solicitation) all copies of Sections I, II, and III of this Questionnaire as provided to assessors.
· The Assessor, in turn, will verify Sections II through III, review Section IV, and complete Section V of this Questionnaire. The assessor will submit the completed Questionnaire to the Marine Corps Installations National Capital Region – Regional Contracting Office.

Your assistance is requested by the USMC to assist with establishing the performance history for the Contractor named below. In efforts to expedite receipt of the requested information, the Contracting Office respectfully requests that you do not mail hard copies. Instead, please e-mail the completed questionnaire(s) to: melissa.chabot@usmc.mil and christa.eggleston-sc@usmc.mil with the subject line stating “SMIP RFP M00264-22-R-0008-PPQ” by 05 Jan 2023 @ 10:00 am Eastern Time.
