About this file

This is a Past Performance Questionnaire (PPQ) template provided by the VA Program Contracting Activity Central (PCAC) for use in evaluating contractor past performance on federal contract opportunities. The form is designed to be completed by client references and incorporated into contractor proposals, counting toward the page limitation specified in the solicitation instructions. The PPQ collects detailed contractor information including the offeror name, address, point of contact, work performed percentage, and subcontractor relationships, along with comprehensive contract details such as contract number, type, location, award date, acceptance dates, and pricing information (original implementation price, original contract price, and final contract price with modifications).

The evaluation section requires client references to assess contractor performance across seven key criteria: quality of design and construction standards, schedule compliance and timeliness of deliverables, communication and cooperation with client staff, management effectiveness and personnel continuity, cost and financial management, subcontract management capabilities, and overall performance summary. Evaluators rate performance using adjectival ratings of Outstanding, Above Average, Satisfactory, Unsatisfactory, or Not Applicable, with detailed definitions provided for each rating level. The form also includes a question regarding willingness to hire or work with the firm again. Evaluators must provide their name, position, contact information, role in the project, and signature with date, with completed questionnaires to be returned to the firm requesting them by the deadline specified in the solicitation.
