About this file

This is a Certified Disadvantaged Business Outreach Plan form (Attachment F) from the Oregon Certification Office for Business Inclusion and Diversity (COBID). The document requires proposers to provide information about their certification status and relationships with certified disadvantaged businesses, including minority-owned, woman-owned, service-disabled veteran-owned, and emerging small businesses certified under ORS 200.055.

The form requires proposers to indicate whether they are Oregon certified firms (including DBE, MBE, WBE, SDV, or ESB certifications), list their contractual relationships with Certified Firms over the past two years, and describe any potential subcontracting opportunities for the procurement. If subcontracting opportunities exist, proposers must detail their steps to solicit Certified Firms, estimate the dollar value of subcontracting opportunities, and confirm their willingness to report subcontractor information to COBID. The document emphasizes that the information provided will not be considered in any scored evaluation, but certification and outreach efforts are required as part of the proposal submission process.
