About this file
This document is Attachment A - Legal Requirements Checklist for Utah Retirement Systems (URS) and PEHP Health & Benefits, related to RFP 83399 for a Microfiche Digitization Project. The project involves securely collecting and transporting approximately 130,000 microfiche jackets and converting them into digital images to be transferred to the agency's system. The RFP requires vendors to submit proposal responses with Attachments A and B, with strict communication rules limiting contact to the PEHP Purchasing Manager via a specific email address (RFP83389MICROFICHE@urs.org).
The legal checklist outlines mandatory compliance requirements for potential vendors, including agreement to specific terms such as 90-day termination provisions, privacy and confidentiality protocols, indemnification, insurance coverage, dispute resolution in Utah courts, potential inspection/audit rights, and conflict of interest disclosures. Failure to complete and return the signed legal checklist with the vendor proposal may result in disqualification from the bidding process. The RFP has been posted on Bidnet Direct, and vendors must adhere to strict communication guidelines to maintain the integrity of the procurement process.
