About this file

This document is an Addendum (Number 2) to a Request for Proposal (RFP Number: 25-04) issued by the Los Angeles Community College District for Student Health Services Centers. The addendum specifically revises the scoring criteria for evaluating proposals, modifying the threshold for recommendation to the Board of Trustees. Originally, proposals needed a score of 75 or higher to be recommended for placement on the qualified providers bench. The revised criteria now require proposals to receive an accumulated score of 65% or higher across three key categories: qualifications/experience/references, staffing/organization, and work plan.

The addendum emphasizes that proposers must acknowledge receipt of this document in writing and incorporate all addenda into their proposal. Failure to identify received addenda may result in the proposal being deemed non-responsive. The revised scoring approach provides more flexibility for potential providers by lowering the scoring threshold while maintaining a comprehensive evaluation process across multiple critical assessment areas.

1 Need to Adjust the Scoring Criteria narrative for

Section 6.2 – Evaluation Procedures, Second Phase, page 23 of 75.

The Second Phase narrative notes: “… evaluation of the First Phase, proposals with a score of 75 or higher will be recommended to the Board of Trustees for placement on the bench of qualified Student Health Services Centers”.

Revised Narrative.

Proposals must receive an accumulated score of 65% (or higher) in the three categories noted below for recommendation to the Board of Trustees for placement on the bench as qualified providers for the Student Health

Services Centers.

The assessment will be based on the total possible score for the three categories listed in the written proposal (see RFP, Section 6.3):

1. Qualifications, experience and references of proposer

2. Staffing and organization

3. Work plan

Therefore, to be placed on the bench, a Proposal's total score must be 65% or higher in the 3 categories listed above.
