About this file
This is a Sworn Statement on Public Entity Crimes form (Attachment E) required for RFP 25-03 Low-Code Application Development Services submitted to the Florida Department of Citrus (FDOC). The form must be completed and notarized by prospective vendors as part of their proposal submission. Bidders are required to certify their compliance status regarding public entity crime convictions under Florida Statutes Section 287.133. The sworn statement is valid through December 31 of the calendar year in which it is filed, and vendors must inform FDOC of any changes to the information contained in the form prior to entering into a contract exceeding the threshold amount specified in Section 287.017, Florida Statutes for Category Two.
The form requires vendors to indicate one of three statements: that neither the entity nor any of its officers, directors, executives, partners, shareholders, employees, members, or agents active in management, nor any affiliate has been convicted of a public entity crime since July 1, 1989; that the entity or affiliated parties have been so convicted; or that a conviction occurred but a subsequent proceeding before the Florida Division of Administrative Hearings resulted in a Final Order determining it was not in the public interest to place the entity on the convicted vendor list. The form must be signed and sworn before a notary public or other authorized official and submitted to FDOC as a requirement for contract eligibility.
