About this file

This is a Request for Information (RFI-25-03) issued by the City of DeLand, Florida, seeking qualified firms to provide comprehensive Employee Health Clinic services for approximately 708 employees, dependents, and retirees. The requested services encompass acute and episodic care, new employee onboarding, primary and preventive care, chronic condition management, laboratory and pathology services, lifestyle management, prescription dispensing, claims integration with the current carrier, limited occupational health services, care coordination, and wellness programs including health risk assessments, biometrics, portal access, health coaching, and ongoing health promotion. Respondents must submit a detailed overview not exceeding thirty pages addressing firm history (including number of clinic clients, health centers operated, public sector and Florida experience, implementation timelines, and customer service philosophy), staffing information (recruiting and retention methods, credentialing processes, hiring role clarity, and employment classification), contact information, reporting capabilities with sample examples, pertinent licenses and certifications (HIPAA, OSHA, CLIA, GINA, COBRA), and a minimum of three to five professional references. Responses are due by 2:00 PM on June 3, 2025, submitted to Corey McMillen, Purchasing Manager, at the City of DeLand, 120 S. Florida Ave., DeLand, Florida 32720, in the form of two paper copies with an original marked copy plus an equivalent electronic PDF file on USB drive. The question deadline is May 27, 2025, and no pre-proposal meeting has been scheduled. This RFI is exploratory in nature and does not obligate the City to issue a subsequent solicitation, enter into contract negotiations, or purchase services from any respondent.

The City currently maintains an onsite clinic at 750 W. Plymouth Ave., Suite B, DeLand, Florida 32720, with an existing lease extending through September 30, 2029 with two optional two-year renewals. The incumbent provider, My Health Onsite, has operated the clinic since 2010, achieving a 93 percent utilization rate reflecting the City's strong culture of prevention and wellness. Current health coverage is provided through Florida Health Care Plans (FHCP) offering three plans: two HMO plans (TD8 and TD7) and one POS plan (T08), with monthly employee rates ranging from $657.94 to $985.93 depending on plan selection. The RFI does not specify an explicit budget amount or pricing structure, nor does it identify set-asides by contractor type or size. All respondents must comply with E-Verify requirements, provide evidence of workers' compensation insurance, comprehensive general liability and auto liability coverage with limits of not less than $1,000,000 combined single limit per occurrence, and professional liability insurance for errors and omissions of $1,000,000 per claim. Successful contractors must also certify compliance with scrutinized contractors provisions related to boycotts of Israel, Sudan, Iran, Cuba, and Syria as applicable. The City assumes no liability for contractor costs associated with response preparation and submission.
