About this file

This document is an Addendum to a Request for Grant Proposal (RFGP) issued by the Oregon Health Authority (OHA) for 6 Regional Coalition Conveners for the Oregon Commercial Tobacco Prevention Menthol Program. The RFGP seeks proposals from organizations interested in serving as regional conveners to prevent flavored tobacco and menthol use. Applicants can apply for up to 3 regions, listed in order of preference. OHA anticipates awarding approximately $140,000 per year per region. There are no limitations on the types of organizations that can apply, including TOFCO convening organizations. The budget can include stipends for participants, and OHA will outline any spending limitations in the grant agreement.

The RFGP does not specify an annual or project budget, but rather states that OHA anticipates awarding 6 grants in the amount of approximately $140,000 per year, divided between the regions. There are no set-asides mentioned, and the document does not identify any incumbent providers. The funding source is not explicitly stated, but the RFGP is being issued under the provisions of Oregon state law.

Subject: Addendum #4

PURPOSE

The purpose of this addendum is to provide Answers to the Questions asked during the Question and Answer period and the pre-proposal conference.

OHA hereby responds the Questions as follows:

Question 1. What kind or work samples are you looking for? Photos, videos, etc.

Answer 1: Anything you feel will demonstrate your connection to community.

Question 2. How do we consider or inform OHA of the regions or areas we would want to apply for? What if we have a preference with one region over another but experience in multiple regions?

Answer 2: See Addenda 1 which modifies Section 3.3.1 as follows:

Provide an overview of your organization, mission, values. Please include what region(s) are you interested in serving as a convener? Please list only the region(s) that you are familiar with and have experience serving. You can list up to 3 regions in order of preference. (Refer to the regional map on Page 8 of the RFGP).

Also see Section 4.11 for Ranking of Proposers, specifically paragraph 4 which states:

If there are no Proposals for a Region, OHA may negotiate with a neighboring Region to act as the convener for that Region.

RFP OB #S-44300-00008246 OHA RFA #5757-3 tlh

Question 3. Can you please clarify the budget available to support this work? Is it $140,000 for the entire multi-year project period or is it $140,000 per year? Basically, is it annual or the whole project?

Answer 3: See Addenda #2 which clarifies Section 1.1, Paragraph 6 as follows:

Agency anticipates the award of six Grants from this RFGP, in the amount of approximately $140,000.00, per year, divided between the Regions.

Question 4. Are there limitations on the types of organization that can apply?

Answer 4: There is a priority for community-based organizations, but no limitations.

Question 5. What if there is a collaborative relationship between a CBO and an LPHA?

Answer 5: There must be one responsible party for the entire grant agreement.

Describe the partners and how the relationship works in your Proposal. A letter of confirmation of collaboration from your partners would be helpful for the Proposal.

Question 6. Can they use this money to compensate others? Is this budget intended to include stipends for participants or is there other money in the budget for supporting that work?

Answer 6: The budget can include stipends. Any limitations to the spending will be outlined in the grant agreement.

Question 7. Are the TOFCO convening organizations eligible to apply? Or is there a conflict or exclusion to those organizations?

Answer 7: There is not a conflict of interest and TOFCO convening organizations would be able to apply.
