About this file

This file contains the required forms for competitive solicitations issued by Hillsborough County Public Schools (HCPS), which vendors must complete and submit as part of their proposal response to RFP 25079-RFP-MST for Occupational Health Program Services. The solicitation seeks qualified vendors to provide comprehensive occupational health services including random drug testing for current employees, new hire drug testing, occupational physicals across various job classifications, DOT physicals, heavy and light physical labor assessments, fit for duty evaluations, drug and alcohol testing, immunization programs, and specialized medical screenings. The proposal deadline is June 19, 2025, at 3:00 PM, with the initial contract term beginning on the first business day after Board approval and including potential automatic renewals for up to four additional one-year periods. The contract is valued at approximately $700,000 for the initial term, and HCPS plans to award both a primary and an alternate vendor.

The required forms encompass multiple certifications and declarations that all proposers must complete, including a Statement of Qualifications addressing contract defaults, bond forfeitures, bankruptcy filings, litigation status, and regulatory sanctions; a Drug-Free Workplace Certification with preference given to vendors with certified drug-free programs in case of tied bids; a Scrutinized Company Certification confirming the vendor is not listed on any state-identified scrutinized companies lists; a federal Debarment and Suspension Certification (Form AD-1048); a Truth and Accuracy Statement; and a Vendor Affidavit regarding human trafficking and labor coercion compliance under Florida Statutes Section 787.06(13). Payment terms default to Net 45 days from delivery or invoice receipt, with options for Net 21 days via E-Payables or early payment discounts, and acceptance of Purchasing Card (P-Card) payments with varying reporting levels. Vendors must demonstrate a minimum of three years of experience, provide three business references, comply with E-Verify and confidentiality standards, and complete a W-9 Vendor Payment Profile Form. If the vendor's principal place of business is outside Florida, an out-of-state attorney's written opinion on bidding preferences must accompany the proposal. The evaluation process uses a weighted criteria system totaling 100 points, with cost/value at 35 points, overall assessment at 25 points, qualifications and experience at 25 points, and additional points available for Small Business Encouragement Program (SBEP) vendors. The pricing structure allows for unit-based pricing with a maximum 15% cost-plus markup for additional items.
