About this file

This document is a Request for Quote (RFQ) issued by the Illinois Emergency Management Agency (IEMA) for Dosimetery services and equipment. IEMA is seeking fixed pricing on various dosimetry-related goods and services, including annual maintenance and repair of the InLight 200 Automatic Reader system, monthly calibration and quality control sets, and the provision of occupational radiation monitoring dosimetry badges for IEMA and Illinois State Police employees. The RFQ outlines the specific requirements and quantities for each item, and requires vendors to submit their quotes by a specified deadline. Award will be based on the total sum of all proposed pricing.

The RFQ provides detailed pricing instructions, including requirements for valid pricing terms, shipping and handling charges, and prohibitions on price increases. It also reserves the agency's right to reject any or all offers, and to award by item, group, or total. Vendors are required to provide new equipment with a minimum 1-year warranty, and the state is exempt from certain taxes. Partial shipments are permitted in the event of delays on the total order.
