These numbers shall be unique across all temporary dosimeter accounts, i.e., no numbers from the Fermilab in Batavia temporary dosimeters shall ever appear on LBNF-FS temporary dosimeters at any point in time.
3) Dosimeters in the two (2) accounts reserved for area monitoring, QC, etc. shall be assigned five (5) digit numbers as determined by Fermilab's Dosimetry Program Manager.
4) The six (6) accounts containing dosimeters worn by personnel shall have the ability to assign an additional six (6) digit identification number to each participant, corresponding to that individual's Fermilab-assigned CNAS number; this number shall be provided by Fermilab dosimetry personnel where appropriate and shall serve as that individual's unique Dose Reporting ID.
5) The five (5) digit numbers referred to in Items 1, 2, and 3 above, along with the participant names, account numbers, subaccount codes, and wear period start dates, shall be clearly printed on the labels of all whole-body dosimetry badges and, where possible due to the limited space available, extremity dosimeters.
6) Every DOELAP-accredited whole-body dosimeter, personnel or otherwise, shall also have a unique, non-repeating eight (8) digit badge ID number assigned to it; this number shall be clearly printed on the labels of all whole-body dosimeters, and a two-dimensional (2D) barcode version of this number shall also be printed on the labels of all dosimeters to enable scanning devices to read this number. The initial sequences of these barcode numbers shall be determined by Fermilab's Dosimetry Program Manager and provided to the subcontractor.
7) Every DOELAP-accredited extremity dosimeter shall also have a unique, non-repeating badge ID number assigned to it; this number shall be clearly printed on the labels of all extremity dosimeters, and a two-dimensional (2D) barcode version of this number shall also be printed on the labels of these dosimeters to enable scanning devices to read this number. The initial sequences of these barcode numbers shall be determined by Fermilab's Dosimetry Program Manager and provided to the subcontractor.
8) Dosimeter labels or holders shall alternate colors to readily differentiate their quarterly wear periods.
E. Dosimeter Shipments
1) Prior to the start of each quarter, Fermilab dosimetry personnel shall supply the subcontractor with an updated list of participants containing their account numbers, ID numbers, subaccount codes, and badge rack locations or mail stop recipient names and numbers. Rack monitoring dosimetry badges shall be included in this list.
2) Fermilab dosimetry personnel shall also supply the subcontractor with physical badge racks labeled with each participant's name and ID number.
3) All personnel and rack monitor dosimeters that are assigned to badge racks by Fermilab dosimetry personnel shall be loaded onto those badge racks in the proper order by the subcontractor. Dosimeters shall be placed directly onto the labeled badge rack prior to shipment to Fermilab.
4) All personnel and rack monitor dosimeters not assigned to badge racks shall be appropriately packaged according to the mail stop recipient names and numbers in the list supplied by Fermilab dosimetry personnel. The names and mail stop numbers associated with those dosimeters shall be written on the outside of their packaging by the subcontractor.
5) Should any dosimeters have no position on a rack nor any entry in the participant list with mail stop name and number, the subcontractor shall ship those dosimeters to Fermilab in bulk.
6) Extremity dosimeters shall be shipped in bulk.
7) Dosimeters from the account reserved for temporary dosimetry badges shall be packaged by the subcontractor and shipped in bulk. These shall be numerically sorted and attached to a suitable carrier to prevent mixing.
8) Dosimeters from the accounts reserved for area monitoring, QC, etc. shall be packaged appropriately by the subcontractor and shipped in bulk.
9) Electronic copies of the packing lists for each account shall be provided for each shipment of dosimeters. Where applicable, the packing list entries shall include account numbers, subaccount codes, dosimeter wear dates, badge ID numbers, and participant names, numbers, and Dose Reporting ID numbers.
10) Additional dosimeters shall be supplied by the subcontractor upon request at any point during a quarter. These shall be shipped in bulk, and electronic copies of the packing lists shall be provided.
11) Dosimeters shall be shipped to ensure arrival at Fermilab at least ten (10) working days prior to the start of the quarter. At the beginning of each year, Fermilab dosimetry personnel will contact the subcontractor to specify anticipated dates for badge racks to be shipped to the subcontractor and returned to Fermilab. Specific shipping dates shall be coordinated between the subcontractor and Fermilab dosimetry personnel around the time the shipment is ready each quarter.
12) Control dosimeters shall be supplied by the contractor with each shipment of dosimeters. The number of control dosimeters needed for each quarterly shipment shall be determined by Fermilab's Dosimetry Program Manager and may be modified at any time by a written request to the subcontractor. For subsequent smaller shipments during a quarter, the subcontractor shall determine the appropriate number of control dosimeters to include.
13) The quarterly shipments of the racks and bulk dosimeters shall be shipped by the subcontractor and delivered to a location at Fermilab in Batavia as specified by the Dosimetry Program Manager. The subcontractor shall ensure that all dosimetry trunks and boxes are to be unloaded from the delivery vehicle by the driver and not by Fermilab personnel.
F. Processing
1) A positive system for identifying and tracking all dosimeters through the processing cycle shall be established by the subcontractor.
2) All dosimeters that are shown to have an exposure of greater than 1 rem or any neutron exposure with no accompanying gamma exposure shall be re-read, and their results shall be examined by a health physicist to have the initial reading verified.
3) All whole-body dosimeters shall have their neutron components processed and their neutron doses reported. If any neutron components are unable to be reported, a list containing, at minimum, the participant names, participant numbers, and wear periods of those dosimeters shall be provided in writing to the Fermilab DPM by the subcontractor.
4) All participant dose results shall be calculated using an algorithm compliant with the DOELAP standards referenced in Section II and consistent with the algorithm used during DOELAP performance testing (see Section VI. C).
5) Any changes to the established algorithms used to calculate doses shall be communicated to Fermilab's DPM in writing.
6) Fermilab dosimetry personnel shall have the option of selecting customer-specific calibration sources and associated correction factors. These selections shall be possible to make at the account level or for any specific subaccount, participant, or individual dosimeter. Fermilab's current selections for calibration sources and correction factors are as follows:
Dosimeter Calibration Source Correction Factor

Fermilab dosimetry personnel shall communicate any future changes to these calibration sources to the subcontractor in writing.
7) The subcontractor shall provide an emergency processing option, with reports to be issued no more than 24 hours after receipt of dosimeters at the subcontractor. The subcontractor shall provide written instructions explaining how to request emergency processing along with pricing for such services.
8) The subcontractor shall process blind audit dosimeters, which will have been irradiated in known radiation fields to known doses by Fermilab dosimetry personnel or a third party. Fermilab dosimetry personnel shall make the results of these blind audits available to the subcontractor. As necessary, performance issues related to the results of these blind audit tests will be discussed during meetings between Fermilab dosimetry personnel and the subcontractor.
G. Reporting
1) The subcontractor shall immediately notify Fermilab's Dosimetry Program Manager in writing when any dosimeter indicates that a dose equivalent equal to or greater than any of the following were received:

• Processor and accredited program identification codes, as appropriate
• Explanations of any deviations from routine processing procedures if the deviation could affect the reported dose
• Signature of or reference to the technical lead (however named)
• Software version(s) of the dose algorithm(s) used
g. Where initials or other abbreviations are used to identify the technical lead (however named) on reports, the subcontractor shall provide Fermilab dosimetry personnel with a means of identifying the full names of the individuals from those initials or abbreviations. This does not have to appear on the reports themselves but can take the form of a reference sheet or equivalent; updated versions shall be provided as necessary.
h. Reports shall be made available to Fermilab dosimetry personnel no more than twenty-five (25) working days after a shipment of used dosimeters is received by the subcontractor for processing.
i. Dose results shall by default be reported in 10 mrem increments unless specifically requested otherwise by Fermilab dosimetry personnel.
j. The symbol SL shall be used to designate a dose equivalent below Fermilab's selected reporting limit of 10 mrem.
k. Control dose values used for control subtractions shall be indicated on all reports. When control dosimeters are omitted from a return shipment of used dosimeters from Fermilab or are otherwise unavailable to the subcontractor, an alternative method of control subtraction shall be used;
this method shall be clearly indicated on the affected report(s), as shall the control dose used.
l. The option shall also be provided for Fermilab dosimetry personnel to request in writing that certain accounts and/or subaccounts not subtract control dose values automatically but instead always report control-unsubtracted values; in this case, the subcontractor shall always automatically provide Fermilab dosimetry personnel with any relevant control dosimeter readings even though they were not used for subtractions.
m. Whenever reports are modified or corrected by the subcontractor, updated versions shall be issued that clearly indicate what changes or modifications were made.
n. Dosimeters returned to the subcontractor late, i.e., beyond ninety (90) from the end of the wear period, shall have their reported doses credited to the proper wear period and participant upon processing.
o. Dosimeters sent back to the subcontractor labeled as Unused shall not have their dose results appear on reports. The data field on the report that normally contains the participant's dose shall indicate that the dosimeter was Unused for that wear period. Should Fermilab dosimetry personnel determine that a dosimeter was incorrectly designated as Unused, the subcontractor shall adjust the participant's dose records accordingly and issue a corrected report.
3) Annual Reports
a. Each year, the subcontractor shall provide electronic summaries of annual and lifetime dose totals for all individuals issued a dosimeter in the preceding year. These data are to be used by Fermilab dosimetry personnel to generate annual dose reports to individuals and to the Department of Energy. This data shall be made available securely through the use of password protection or equivalent means.
b. Annual reports shall be provided for each specific facility separately as designated by that facility's CAIRS code. Personnel monitored for radiation at both Fermilab in Batavia and LBNF-FS shall appear separately in each annual report with their dose received at each facility specified.
c. Lifetime reports shall be provided that combine dose for each individual from all facilities and accounts into a single record for that individual, e.g. if someone was monitored in a given year at both Fermilab in Batavia and LBNF-FS, the total dose from both facilities shall be combined into a single entry for that individual in the lifetime report.
H. Spare Dosimeters
1) As noted in Item V. C 1 above, two (2) accounts shall be dedicated to spare dosimeters. Each dosimeter in this account will either be issued to a participant by Fermilab personnel or returned to the subcontractor as Unused; see Item V. G 2(o) for information on Unused dosimeters.
2) For spare dosimeters that were issued, Fermilab dosimetry personnel shall enter the data for those participants in the subcontractor's online database where possible, e.g. for participants who have never been issued a dosimeter at Fermilab, or who have only ever received spare dosimeters in the past. When a participant is issued subsequent spare dosimetry badges, their doses shall be combined into that participant's dose history record in the spare dosimeter account. As with all accounts for dosimeters issued to personnel, the subcontractor shall keep quarterly dose records and lifetime exposure histories for the participants in this spare dosimeter account.
3) In situations where a participant who was issued a spare dosimeter already exists in one of the other "permanent" accounts, Fermilab dosimetry personnel must send the spare dosimeter and participant information to the subcontractor in writing. The subcontractor shall then merge the dose data from the spare dosimeter with the pre-existing dose data from that participant's entry in their home account. This data shall be reported on the dose reports for the participant's home account.
I. Backup Services and Equipment
1) The subcontractor shall have a documented plan in place to maintain adequate backup equipment and systems for use in the event their primary systems fail.
2) Backup equipment and systems must be routinely tested by the vendor to demonstrate its capabilities.
