About this file
This document is a submissions checklist for a Request for Response (RFR) issued by the Commonwealth of Massachusetts, Department of Insurance (Division of Insurance), for Third Referee Appointee services. Bidders may respond to one or both of two service categories: Third Referee Services for Review of Fire and Casualty Insurance Agent Contract Cancellation, Modification, or Expiration under M.G.L. c. 175, § 163, and/or Third Referee Services for Review of Disputes as to Amount of Loss in Fire Insurance Claims under M.G.L. c. 175, §§ 99-100H. This RFR is a repost intended to replace previous bid BD-27-1027-1FED-1FED-133333. The checklist outlines the sequential order and required content for RFR responses, including business response materials, professional background documentation, general forms, contract forms, and cost proposals.
Bidders must submit a comprehensive response package that includes introductory materials, qualifications documentation with resumes, three business references, contractor authorization and environmental practice forms, Commonwealth terms and conditions, W-9 tax reporting verification, and a standard contract form. Additionally, bidders selected as qualified vendors for specific projects must submit Memorandums of Understanding specific to each service category, a Prompt Pay Discount Form, and authorization for Electronic Funds Payment. The cost proposal must include a cover letter stating the proposal is valid for ninety days from the response due date, accompanied by the completed Cost Proposal Form. The checklist itself must be completed and submitted as the first item in the business response.
