About this file
This is a Recycled Material Content Questionnaire and Affidavit form required by Collin County, Texas, for a Signs, Sign Materials, and Hardware contract opportunity. The form mandates that suppliers provide detailed information about the recycled content of products being procured, demonstrating compliance with the county's preference for environmentally responsible materials. Suppliers must identify which section of the Recovered Materials Advisory Notice (RMAN) applies to their products, specify the percentage of recovered material and post-consumer material content, and confirm whether products meet the Specific Recommendations for Procurement of Designated Items established by the Environmental Protection Agency's Comprehensive Procurement Guideline.
The affidavit requires suppliers to swear or affirm the accuracy of submitted information and obligates them to maintain the stated recycled content levels throughout the contract term. Suppliers must provide documentation verifying material content claims and commit to notifying Collin County within 30 days of any changes to the recyclable content cited in the affidavit. The county reserves the right to audit supplier files to confirm ongoing compliance with recycled material requirements. All signs, sign materials, and hardware items must meet or exceed standards set forth in the Texas Manual on Uniform Traffic Control Devices (TMUTCD) and Texas Department of Transportation (TxDOT) specifications.
