About this file

This document is a Statement of Work (SOW) for a federal contract opportunity to provide Root Cause Analysis (RCA) and Human Factors Analysis Classification System (HFACS) training to the Norfolk Naval Shipyard (NNSY) personnel. The key objectives are to enhance NNSY's capability to investigate, analyze, and expose root causes, including human influences, in operational investigations of incidents and accidents to improve operational performance and safety.

The contract requires the vendor to develop and administer a 3-day curriculum covering RCA and HFACS, delivered in 6 separate training sessions for up to 20 NNSY personnel per session. An additional half-day HFACS facilitated investigation workshop is also required as an immediate follow-up. The vendor must provide certified instructors, training materials, and web-based platform access for 1 year post-training. The contract will be awarded to the responsible offeror whose quote conforms to the solicitation and is most advantageous to the Government, with technical capability and price as the evaluation factors.

Performance Work Statement

Root Cause and Human Factor Analysis Classification Systems Training

Section A: Scope

1. Objectives:

Norfolk Naval Shipyard (NNSY) seeks Root Cause Analysis (RCA) training supplemented by Human Factor Analysis and Classification System (HFACS) training. The overall training will enhance organizational capability to investigate, analyze, and expose root causes including human influences in operational investigations of incidents and accidents to improve operational performance and safety. The training will help safety professionals reduce the accident rate through systematic, data-driven investigative strategies and objective evaluation of mitigation methods. Desired end state is to have participants be equipped with efficient and effective knowledge for investigative insights that will encompass and expand the established NNSY Team Learning Session (TLS) Training. NNSY requires the training to produce a class of students with an ability to clearly understand known causal relationships to determine which causes that if removed or controlled will support an effective outcome and prevent reoccurrence of errors. This training shall equip NNSY personnel with the skills and abilities that will improve timely completion of TLS with accurate reporting.

Additional Objectives and Requirements:

1. NNSY requires built in time for managers to address Opening and Closing remarks of goals and expectations and how those would apply to the current TLS process.

2. NNSY would require from the Vendor/Contractor to tailor training toward behavior conflict and resolution to have key objectives to prevent a mindset of making unwarranted decision that caused the unplanned event.

3. NNSY requires training for an online principle based software that incorporates a mapped path to a final resolution that form root causes.

4. Provide post-training online software access for 1 year for applicants who received certificate of completion of Root Causal Analysis.

2. Background/Introduction

RC and HFACS framework have been used within the military to systematically examine underlying human causal factors and to improve investigations. The development of HFACS was prompted by increasing problems with human performance. Current state of NNSY requires the application of RC and HFAC methodologies in support of process improvement. When NNSY encounters mishaps, near misses, safety non-compliance, injuries etc, it is essential for these incidents to be captured and documented to communicate lessons learned and derive mitigating actions going forward. This is accomplished through Team Learning Sessions, with the expectation that the session be performed immediately following an incident and/or accident. NNSY is currently experiencing a challenge in solving problems and investigating incidents involving human error. Timeliness of the completion of TLS Reports have also presented a challenge. To address these concerns, NNSY requires Root Cause and Human Factor Analysis and Classification Systems training.

3. Breadth and Limitations of Work

The contractor shall hold an accreditation for Root Cause Analysis Facilitator(s) and Human Factor Analysis.

The contractor shall provide an RCA and HFAC certified instructor to administer and facilitate the training.

Section B: Applicable Documents

4. Documentation: N/A

Section C: Requirements

5. Scope of Work:

NNSY requires the contractor to develop and administer a 3-day curriculum covering the subject and application of Root Cause and Human Factors Analysis combined and tailored to NNSY. NNSY requires six

(6) instances of this training performed at six separate timeframes as coordinated between NNSY and the contractor. The training shall be instructor-led by instructor(s) with certification in both Root Cause Analysis, and Human Factor Analysis. The contractor shall provide all necessary and associated instructors, training material and all modules on a web-based platform. The curriculum shall encompass causal analysis system to target root causes and eliminate conjecture upon establishment of key facts in the investigative process. Each training session shall be administered to up to 20 NNSY personnel who investigate, report, and analyze incidents and mishaps. The contractor is required to have the training facilitated by an instructor with experience developing programs for complex investigations that involve human error scenarios.

NNSY requires the contractor to provide an additional half-day HFACS facilitated investigation workshop as an immediate follow-up to the Root Cause and Human Factor Analysis Training. This half-day workshop shall be interactive and instructional. The workshop shall cover and encompass both the Human Factors and Root Causes Analysis using web-based scenarios, material and demonstrate with visuals aids, as applicable.

The goal is that each participant will add to their personal development by applying new additional tools of methodology to become a more effective and efficient investigator for their respective Codes.

6. Contractor Responsibilities:

The contractor shall provide a certified instructor to facilitate the training.

The contractor shall supply all the required material for the training.

The contractor shall provide Training Feedback and Course Critique Forms to participants.

The contractor shall administer Training Completion Certificates to participants.

7. NNSY Responsibilities:

NNSY shall provide a space to facilitate the training and/or workshop.

NNSY shall provide Audio Visual Support if necessary.

NNSY shall facilitate base access.

NNSY shall provide Internet Access via MIFIs
