About this file

This document contains Questions & Answers related to a state contract opportunity issued by the Illinois Secretary of State (ILSOS) for Carbon Interleaved Forms (B-41776). The RFQ stipulates a partial delivery of the forms by 3/15/24 and the remainder by 3/31/24, despite a vendor's request for a longer 75-day timeframe. The ILSOS has indicated that crash numbering of the forms would be acceptable, contrary to the original specification. The ILSOS has also provided a PDF document with photos to illustrate how the forms should be stitched in the stub.

The document addresses pricing, quantity, and other commercial terms related to the contract. It indicates that the ILSOS has an immediate need for the forms and must hold firm on the delivery dates outlined in the RFQ. There is no mention of any set-asides, incumbents, or funding sources, as this appears to be a standard procurement for the state.

Questions & Answers Carbon Interleaved Forms B-41776

1. The 12# bond is running on a longer time frame. May take up to 75 days. Would this be ok? ILSOS has an immediate need for these forms. The RFQ states a partial delivery by 3/15/24 and the rest by 3/31/24. At this time, we need to hold firm on those dates.

2. We only crash number. The first number is printed, but the others are crash. Is that ok? The department has indicated that crash numbering would be acceptable. You can disregard that specification.

3. Is there a sample you can show so we can see how it is stitched in the stub? Were fillers used to build up the stub? A PDF document with photos has been added to the attachments tab.
