About this file

This is a Quality Assurance Surveillance Plan (QASP) document for medical courier services contracted to transport laboratory test specimens for the Veterans Affairs Sierra Nevada Health Care System (VASNHCS). The QASP establishes performance standards and monitoring mechanisms to ensure the contractor meets service requirements throughout the contract period.

The QASP defines quality standards across multiple performance areas including on-time delivery of specimens, proper handling and temperature control of samples, vehicle maintenance and cleanliness, driver qualifications and conduct, and documentation accuracy. Performance metrics include delivery timeliness standards (specific time windows for pickup and delivery), specimen integrity requirements (maintaining proper temperature ranges and preventing contamination), and compliance with all applicable federal regulations. The plan specifies monitoring activities conducted by the Government, including regular inspection of vehicles and equipment, review of delivery records and chain-of-custody documentation, customer satisfaction assessments, and performance data analysis. Remedial actions and corrective measures are outlined for instances where the contractor fails to meet established performance thresholds, with potential consequences ranging from written notification to contract termination for repeated or severe deficiencies. The QASP functions as the primary tool for Government oversight and quality assurance throughout contract execution, establishing clear expectations and objective criteria for evaluating contractor performance in transporting time-sensitive medical specimens.

1. PURPOSE

This Quality Assurance Surveillance Plan (QASP) provides a systematic method to evaluate performance for the stated contract. This QASP explains the following:

· What will be monitored.

· How monitoring will take place.

· Who will conduct the monitoring?

· How monitoring efforts and results will be documented.

This QASP does not detail how the contractor accomplishes the work. Rather, the QASP is created with the premise that the contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance.

This QASP is a “living document” and the Government may review and revise it on a regular basis. However, the Government shall coordinate changes with the contractor. Copies of the original QASP and revisions shall be provided to the contractor and Government officials implementing surveillance activities.

*Note. The QASP is not a part of the contract. Its sole use is to monitor contractor performance.

Performance measures Performance measures are comprised of performance indicators (some characteristic of a deliverable that can be measured) and performance measures (a mark, measure or benchmark that government personnel use as a point of comparison when evaluating contractor performance). The Government performs surveillance to determine if the contractor exceeds, meets or does not meet these measures.

The Performance Requirements Summary Matrix, paragraph <enter number> in the Performance Work Statement (PWS), includes performance measures. The Government shall use these measures to determine contractor performance and shall compare contractor performance to the Acceptable Quality Level (AQL).
