About this file
This is a Past Performance Questionnaire (PPQ) template designed for evaluators to assess contractor performance on projects relevant to the VA's CFM Healthcare Architect-Engineer IDIQ contract (36C10F26R50011). The form is completed by past clients of firms bidding on this solicitation and serves as a pre-decisional evaluation tool to support the SF330 architect-engineer qualification submission process.
The questionnaire is divided into two main sections. Section 1 requires the contractor to provide contract information including firm name, address, point of contact details, work performed percentage, contract title, contract number, contract type, award and acceptance dates, original and final contract prices, and project scope description. Section 2 is completed by the evaluator (past client) who rates the contractor's performance across seven evaluation categories: Quality (ability to meet design and construction standards), Schedule/Timeliness (compliance with delivery schedules and liquidated damages), Communication (cooperation and responsiveness), Management/Personnel/Labor (effectiveness of project leadership and personnel continuity), Cost/Financial Management (ability to meet contractual pricing), Subcontract Management (subcontracting oversight and problem resolution), and Summary (overall recommendation and performance rating). Evaluators use a four-point adjectival rating scale: Outstanding, Above Average, Satisfactory, and Unsatisfactory. Completed questionnaires are returned to the offeror for incorporation into the SF330 submission rather than submitted directly to VA CFM, and the rating definitions page is removed from the final SF330 page count.
