ATTACHMENT C – PROPOSAL CHECKLIST
This proposal checklist is to assist Offerors in ensuring that proposals are complete. An electronic copy of this completed checklist must be included with all offers submitted. Prior to preparation of your offer, be sure to read the entire solicitation. Offers shall be prepared and submitted in the format outlined below.

d. Process for vehicle shipment
e. Ability to comply with standard delivery which conforms to solicitation requirements
f. Project Plan identifying the following:
1) Date of completion or proposed build order
2) Order placement with all significant subcontractors, if applicable (cab and chassis, body, etc) For non-OEM offerors, identify order placement procedures with suppliers
3) Delivery date of subcontracted items
4) Planned completion timeline
5) Planned inspection and shipment timeline
g. The time of shipment specified for each representative line item is supported by the Project Plan
h. No unapproved exceptions taken to the Federal Standards
i. Does not exceed 25 pages, excluding technical exceptions to the Federal Standard 833K (Attachment D)
Tab B – Exceptions to Federal Vehicle Standard 833K (Attachment D)
Factor 2 – Management Capabilities (15 Pages Maximum)
Tab A – Management Approach
a. Timely order processing that conforms to solicitation requirements for the time of shipment specified for each representative line item
b. Oversight to ensure production, shipment and delivery of vehicles conform to solicitation requirements (includes methods and timetables for certificates of origin and other vehicle documentation)
c. Management of suppliers and dealers
d. Warranty administration
Tab B – Damage in Transit Procedures
e. DIT procedures identifying the following:
1) Protocol for customers to report damage/apparent missing equipment
2) Contact information
3) Notification deadline
4) Resolution procedures
f. Does not exceed 15 pages.
Factor 3 – Past Performance (5 Pages Maximum)
a. Projects: The work described in the projects provided must demonstrate the successful completion of work which was of a similar or greater complexity to the work described in the specifications and proposed to be accomplished by the offeror
b. Does not exceed 5 pages
