About this file
This is a Quality Assurance Surveillance Plan (QASP) for snow removal and deicing services at the Wilmington VA Medical Center in Wilmington, Delaware. The document establishes performance standards, acceptable quality levels (AQLs), surveillance methods, and corrective action procedures that the contractor must meet during contract performance. The QASP is aligned with the Performance Work Statement (PWS) and defines the roles of the Contracting Officer (CO), Technical Point of Contact (TPOC), and VA Police/Engineering Service in oversight and performance validation.
The contract requires five core performance areas with specific metrics: (1) Timeliness of Response—100% compliance with 1-hour mobilization and 15-minute emergency callbacks, with zero emergency deviations and a maximum of one non-emergency deviation per month; (2) Snow & Ice Removal Quality—95% overall compliance and 100% in critical areas, with continuous hazard mitigation and clearing by 6:00 AM daily; (3) Critical Area Maintenance—100% compliance for priority clearing of ADA-accessible areas, emergency routes, and fire hydrants with no unsafe accumulation; (4) Garage Compliance—100% compliance using only approved chemicals and prohibiting metal blades; and (5) Documentation Accuracy—100% compliance with zero errors on Snow Control Sheets and National Weather Service data. Surveillance methods include daily inspections, real-time monitoring, equipment inspections, and invoice reviews. Invoices must include TPOC-validated Snow Control Sheets and NWS accumulation data for Mount Holly, New Jersey. The contractor must maintain an internal Quality Control Program, and the COR may document deficiencies and impose immediate corrective actions, with repeated or significant issues resulting in contract remedies or negative performance evaluations.
