PERFORMANCE BASED

QUALITY ASSURANCE SURVEILLANCE PLAN

MISO EVALUATION AND TRANSREGIONAL ANALYSIS (META)

TASK ORDER ID: ID04200022

1. PURPOSE:

This Quality Assurance Surveillance Plan (QASP) provides a systematic process for the Government to evaluate performance for the stated contract. This QASP explains the following:

· What will be monitored

· How monitoring will take place

· Who in general will conduct the monitoring

· How monitoring efforts and results will be documented

The QASP is created on the premise that the contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance.

2. GOVERNMENT ROLES AND RESPONSIBILITIES:

The following personnel shall oversee and coordinate surveillance activities.

a. Senior Contracting Officer (SCO) - The SCO ensures performance of all necessary actions for effective contracting, ensures compliance with the contract terms, and shall safeguard the interests of the Government in the contractual relationship. The SCO shall also ensure that the contractor receives impartial, fair, and equitable treatment under this contract. The SCO is ultimately responsible for the final determination of the adequacy of the contractor’s performance.

b. Contract Specialist (CS) - The CS acts as an acquisition consultant and serves as liaison between the General Services Administration (GSA), the client (USSCOM MISO), and the contractor.

c. Contracting Officer’s Representative (COR) - The COR is responsible for technical administration of the contract and shall ensure proper Government surveillance of the contractor’s performance either directly or preferably by delegation to the appropriate responsible or assigned Government Technical/Functional Representative. The COR keeps a record of quality assurance (QA) related documentation to include technical information, and other relevant documentation or correspondence to include Internal Office Correspondence (IOCs) and verbal discussions or telecoms minutes. The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government’s behalf. The contractor shall refer to the SCO for action for any changes they determine may affect contract price, terms, or conditions.

3. PERFORMANCE STANDARDS:

Performance standards define desired services. The Government performs surveillance to determine if the contractor exceeds, meets, or does not meet these standards.

The contractor shall provide episodic support for known and emerging requirements to support USSOCOM META operations to include collection, storage, formatting, analyzing and presentation of information. The contractors emerging requirement support shall assist the USSOCOM J39 in its MISO requirements. Contractor shall be capable of providing requested support starting no more than 14-days from notification of the KO.

The contractor shall provide all required personnel to successfully perform the work detailed herein. The contractor shall address all performance issues and customer complaints to ensure the requirements defined in the PWS are met successfully 100% of the time.

Contractor met requirements in the PWS and received no more than two written customer complaints/notification of non-conforming services during a single one-year performance period AND complaints/non-conforming services were successfully corrected by the contractor within seven calendar days of notification.

The contractor shall provide the data deliverables in a contractor provided format ensuring information contained therein completely covers the requirements of the deliverable.
Contractor met the requirements in the PWS and received no more than two factual/accuracy errors during a single one-year performance period AND all rejected data deliverables were corrected by the contractor within two business days of notification of Government rejection.

4. METHODS OF QA SURVEILLANCE:

The Government will evaluate the performance objectives through surveillance as reflected above by reviews and acceptance of work products and services.

5. STANDARD

The contractor shall perform all work required in a satisfactory manner in accordance with the appropriate PWS paragraph. The services should not be considered complete until all deficiencies have been corrected. The COR shall notify the SCO for appropriate action if any of the above performance objectives are not in accordance with acceptable AQL.

6. PROCEDURES

Inspection and acceptance will occur in accordance with FAR 52-212-4 Contract Terms and Conditions-Commercial Items. In the absence of other agreements negotiated with respect to time provided for Government review, deliverables will be inspected and the contractor notified of the findings within seven calendar days of normally scheduled review. If the deliverables are not acceptable, the COR will notify the contractor immediately. Acceptance of invoices shall constitute acceptance of performance. Performance by the contractor to correct defects found by the Government as a result of quality assurance surveillance and/or by the contractor as a result of quality control shall be at the contractor’s own expense without additional reimbursement by the Government. Unless otherwise negotiated, the contractor shall correct or replace all non-conforming services or deliverables not later than seven calendar days after notification of non-conformance.

The Contractor shall be responsible for overall responsiveness, cost control, adherence to schedules, responsible and cooperative behavior, quality of work, subject matter expertise, and understanding of the requirements herein.

The COR will document all defects using a “Task Discrepancy Report (TDR)”. The COR will verbally notify the GSA CS or SCO of the defect and will submit a copy of the TDR as a Post Award Collaboration in ITSS to the GSA SCO. The COR will ensure the TDR is completed in its entirety and coordinated through the SCO. The contractor will also be furnished a copy of the TDR. The contractor shall furnish their response to the TDR. The contractor’s response will address the accuracy and validity of the defect, planned corrective action and actions to prevent recurrence. The TDR information will be included when assessing the contractor’s overall performance (i.e., past performance assessments, exercise of options, etc.).

If any of the services do not conform to the contract requirements, the Government may require the contractor to perform the services again in conformity with contract requirements. The contractor must correct the defect(s) within the timeframes established above. The COR shall not certify satisfactory performance for the contract until all defects have been corrected.

When the defects in services cannot be corrected by re-performance, the Government may:

1) Require the contractor to take necessary action to ensure that future performance conforms to contract requirements; and,

2) Reduce the contract price to reflect the reduced value of the services performed.

The COR shall, in addition to providing documentation to the SCO, maintain a complete quality assurance file. The file will contain copies of all reports, evaluations, recommendations, and any actions relating to the Government’s performance of the quality assurance function, including originals of all surveillance activity checklists. All such records will be retained for the life of the contract. The COR shall forward these records to the GSA SCO at completion or termination of the contract.

The COR shall upload a copy of the Surveillance Log as an Post Award Collaboration in ITSS addressed to the GSA CS and SCO at least once every three months.

ACCEPTANCE OF SERVICES

Acceptance of services shall be based upon compliance with performance standards described in the PWS and surveillance procedures described in this QASP. Prior to approving/certifying any contractor invoices, the COR will verify that all invoiced services have been performed in compliance with contract requirements. The COR shall not certify satisfactory performance for the contract until all defects have been corrected.
