About this file

This is a Quality Assurance Surveillance Plan (QASP) for the repair or replacement of Generator 4A Switchgear at VA Connecticut Healthcare System facilities in West Haven and Newington. The document establishes the framework for monitoring and evaluating contractor performance throughout the contract period, defining roles, responsibilities, and measurable standards for quality assurance.

The QASP outlines five key performance standards with specific Acceptable Quality Levels (AQLs): successful completion of repair/replacement work per schedule and the Performance Work Statement (95% timely performance), timely submission of deliverables such as quotes, materials, work plans, and compliance certifications (95% compliance), full compliance with NFPA, OSHA, EPA, Massachusetts DEP, and VHA standards (100% compliance), proper site management including cleanliness and safety protocols (100% compliance), and timely resolution of customer issues (95% timely resolution). The Contracting Officer's Representative (COR) will conduct surveillance through periodic inspections and review of submittals, with quarterly performance assessment meetings required. The plan establishes a rating scale of "Exceeds Standards," "Meets Standards," or "Does Not Meet Standards," with documentation procedures for both acceptable and unacceptable performance. Unacceptable performance triggers formal written notification via Contract Discrepancy Report (CDR) and may require submission of a corrective action plan. All performance metrics are tracked for potential entry into the Contractor Performance Assessment Reporting System (CPARS).
