About this file

This document is a Quality Assurance Surveillance Plan (QASP) for grounds maintenance services at Fort Sill Commissary. The QASP establishes the systematic framework for evaluating contractor performance against standards defined in the Statement of Work, including assessment methodologies, performance standards, and roles and responsibilities for Government personnel and the contractor.

The Contracting Officer retains ultimate contract authority and can modify terms, resolve disputes, and take contractual action based on QASP findings. The Store Director/Manager (MGR) conducts routine surveillance through scheduled and unscheduled walk-throughs (minimum twice monthly), inspects site work, documents results, and communicates performance issues to the contractor. The Store Director provides user-level feedback and activates Quick Response Service as needed. The Contractor Site Manager represents the contractor on-site and ensures SOW compliance with prompt response to MGR inquiries and re-performance requests.

Performance metrics and Acceptable Quality Levels (AQL) include: Routine Grounds Maintenance (grass mowed to 3 inches, edges trimmed, clippings and debris removed with no more than two valid defects per month); Mulching and Shrub Pruning (uniform application, natural pruning shape, zero defects); Chemical Application (on-schedule applications using Government-approved non-toxic chemicals with zero safety or environmental violations); Quick Response Service (48-72 hour response with completion within 8 labor hours); and Storm Water/Environmental Compliance (zero environmental violations). Surveillance methods include periodic inspection, 100% inspection for specific tasks, customer complaint documentation, and contractor documentation review.

Non-performance remedies include verbal or written notice to the contractor, required correction within 24 hours at no additional Government cost, issuance of a formal Contract Discrepancy Report or Corrective Action Request for severe or repeated defects, and potential contractual actions including negative Past Performance (CPARS) evaluations, payment deductions based on Contract Line Item Number value, or termination for cause.

Quality Assurance Surveillance Plan (QASP)

Grounds Maintenance Services – Fort Sill Commissary

1. Purpose

This Quality Assurance Surveillance Plan (QASP) provides a systematic method to evaluate Contractor performance for the Fort Sill Commissary Grounds Maintenance contract. This document explains the assessment methodologies, performance standards, and roles and responsibilities to ensure the Government receives the level of service defined in the Statement of Work (SOW).

2. Roles and Responsibilities

Role Responsibilities

Contracting Officer (KO)

Retains the ultimate authority for administering the contract. Only the KO can modify the contract terms, resolve formal disputes, or take contractual action against the Contractor based on QASP findings.

Sore Director

(MGR)

Responsible for routine surveillance, inspecting Contractor performance, documenting results, and communicating with the Contractor regarding performance issues. The MGR does not have the authority to alter the contract.

Store Director (Customer) Provides user-level feedback and activates the "Quick Response Service" as needed.

Contractor Site Manager

Represents the Contractor on-site, ensures all SOW requirements are met, and responds promptly to MGR inquiries and KO requests for re-performance.

3. Surveillance Methods

The Government will use the following methods to monitor the Contractor's performance:

1. Periodic Inspection: The MGR will conduct scheduled and unscheduled physical walk-throughs of the Commissary grounds (minimum of twice per month) to assess routine maintenance, mowing height, edging, and general cleanliness.

2. 100% Inspection: Used for specific, easily measurable tasks. The MGR will inspect the site after every execution of "Annual Mulching," "Seasonal Shrub Pruning," and "Quick Response Service" events.

3. Customer Complaints: The MGR will document and investigate valid complaints received from the Commissary staff or patrons regarding grounds appearance or safety hazards (e.g., debris left on walkways).

4. Documentation Review: The MGR will review the Contractor's chemical application logs and MSDS sheets to ensure compliance with Fort Sill environmental regulations.

4. Performance Requirements Summary (PRS)

This table outlines the key performance metrics, the Acceptable Quality Level (AQL), and the method of surveillance for each major service area.

SOW Reference Performance Metric Acceptable Quality Level (AQL) Surveillance Method

Routine Grounds Maintenance

Grass mowed to 3 inches; edges trimmed neatly; all clippings and yard debris removed from walkways and drains.

No more than two (2) valid defects observed per month.

Periodic Inspection / Customer Complaint

Mulching & Shrub Pruning

Mulch applied uniformly; shrubs pruned to natural shape without obstructing walkways.

Zero (0) defects; work must be completed exactly as scheduled.

100% Inspection upon completion

Chemical Application (Fertilizer/Pesticide)

Applications performed on schedule using only Government-approved, non-toxic chemicals where feasible.

Zero (0) safety or environmental violations.

Documentation Review / Periodic Inspection

Quick Response Service

Contractor responds within 48-72 hours of notification and completes directed tasks within the 8-labor-hour limit.

100% compliance with response time and labor hour constraints.

100% Inspection / Verification by Store Director

Storm Water / Environmental Compliance

No yard waste, clippings, or chemical runoff enters storm drains.

Zero (0) environmental violations.

Periodic Inspection / Customer Complaint

5. Remedies for Non-Performance

If the Contractor fails to meet the Acceptable Quality Level (AQL) for any metric, the Government will take the following steps:

1. Notice to Contractor: The MGR will verbally or in writing notify the Contractor's Site Manager of the defect.

2. Re-performance: The Contractor must correct the deficient work within twenty-four (24) hours of notification at no additional cost to the Government.

3. Documentation: If the defect is severe, poses a safety risk, or if the Contractor fails to re-perform the work within 24 hours, the KO will issue a formal Contract Discrepancy Report (CDR) or Corrective Action Request (CAR).

4. Contractual Action: Repeated failures to meet the AQL, or failure to address CARs, may result in negative Past Performance (CPARS) evaluations, deductions in payment (calculated based on the CLIN value of the unperformed work), or termination for cause by the KO.
