About this file

DA Form 2404 is a Department of the Army Equipment Inspection and Maintenance Worksheet used to document equipment condition assessments and maintenance activities. The form serves as the official record for all inspections and equipment conditions determined in accordance with diagnostic procedures and standards referenced in applicable Technical Manuals (TMs). It is governed by DA PAM 750-8, with the proponent agency being DCS, G-4.

The form contains five primary columns for recording inspection data: Column A for TM item numbers, Column B for applicable condition status symbols, Column C for deficiencies and shortcomings, Column D for corrective actions, and Column E for initials by the individual ascertaining completed corrective action. Status symbols include "X" for inoperable deficiencies, circled "X" for deficiencies allowing limited operation, horizontal dash "(-)" for overdue inspections or maintenance, diagonal slash "/" for material defects affecting efficiency, and last name initial in black or blue-black ink for satisfactory conditions. For aircraft, status symbols are recorded in red. The form also captures organizational information, equipment nomenclature and model, registration/serial/NSN, operational metrics (miles, hours, rounds fired, hot starts), inspection date, inspection type, applicable TM references, personnel signatures, and required manhours. A sample populated section includes seven standard maintenance items: greased and lubricated vehicle, checked all fluids, checked lights, cleaned battery, checked tire PSI, tested anti-freeze, and other items.
