The Junior Contract Analysts must possess a bachelor's degree with at least 24 semester hours in accounting, business, finance, law, contracts, purchasing, economics, industrial management, marketing, quantitative methods, or organization; minimum two years of federal government or contractor experience in the GS-1102 series or equivalent contracting specialist role; DAWIA Contracting Level I certification; knowledge of federal acquisition processes, contracting principles, laws, regulations, and procedures; and U.S. citizenship. Duties include accomplishing pre- and post-award documentation in compliance with FAR and DFARS regulations; providing acquisition lifecycle support in purchasing, source selection, training, research, business process improvement, and program management; formulating Performance Work Statements and requirements packages; coordinating with division directors and contracting offices; conducting training and briefings; assisting CORs with programmatic guidance; and tracking contractor credentials and security requirements. Work shall be performed Monday through Friday, 7:30 AM to 4:00 PM, with a 30-minute unpaid lunch break, limited to 80 hours per two-week period and 12 hours per 24-hour period. Overtime is not authorized. The contractor must provide all personnel, equipment, supplies, materials, supervision, and non-personal services necessary for performance, excluding government-furnished items. Government furnishes facilities, utilities, equipment (telephones, facsimile machines, copiers, computers, laptops), training and orientation, and administrative materials. The contractor is responsible for maintaining a Quality Control Plan, ensuring employee qualifications and training compliance, conducting security investigations and background checks, managing personnel records and timekeeping, coordinating absences with the government site supervisor and COR, and ensuring all health and immunization requirements are met. Key performance metrics include overall fill rate (>82%), turnover rate (<25%), on-time fill percentage (>85%), replenish rate (>80%), customer satisfaction (>3.0 on a 5-point scale), complaint resolution (90%), and package acceptance percentage (>92%), measured monthly and reported to the COR by the 5th calendar day of each month.

Department of Defense Defense Health Agency Performance Work Statement

Contract Management Support At A.T. Augusta Military Medical Center Fort Belvoir, VA

PART 1

1.0 GENERAL INFORMATION

1.1 This is a non-personal services contract to provide Contract Analyst Support for A.T. Augusta Military Medical Center (ATAMMC), Fort Belvoir, VA . This non-personal services contract under which the contractor rendering the services is not subject, either by the contract’s terms or by the manner of its administration, to the supervision and control usually prevailing in relationships between the government and its employees. The government will not conduct interviews.

1.2 Description of services/introduction: The contractor shall provide all personnel , equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform concierge services as defined in this Performance Work Statement (PWS) except for those items specified as government furnished property and services. The contractor shall perform to the standards in this performance work statement. PWS.

ATAMMC:

• Junior Contract Analyst (2 FTE)

1.3 Background: The contract for contract analyst support services was established in 2020 to ensure continued services to Department of War (DoW) beneficiaries at ATAMMC. The contract will work in conjunction with other healthcare professionals and non-contract personnel to develop/capture ATAMMC’s contract requirements.

1.4 Objectives: The objective of this effort is to provide Directorate of Resource Mananagnent, Contract Support. The Contractor shall provide 2 FTEs Junior Contract Analysts to support Contract operations. The Contractor shall perform the required services in accordance with (IAW) the terms and conditions of this contract and the included Performance Work Statement (PWS). Overtime is not authorized.

1.5 Scope: Contract Management Support. Services include but are not limited to, acquisition and contract management, program management support, and other related workload requirements associated with the acquisition and administration of awarded contracts. The contractor shall accomplish a variety of acquisition and other related administrative services to complement the Government’s workplace capabilities. The Contractor shall provide qualified personnel to provide services to Alexander T Agusta Military Medical Center and all outlying clinics. Performance will be base year with four options years, starting 30 Dec 2026 – 29 Dec 2027.

1.5.1. Contractor personnel shall be fluent in reading, writing, and speaking English. Contractor personnel shall use English as the primary language while on duty, except when assisting non-English speaking individuals

1.6 Period of Performance (PoP): The period of performance shall be for one (1) Base Year and four (4) 12-month option years. The Period of Performance reads as follows:

1.7 Administrative specifications:

1.7.1 Place of performance: The work shall be performed at ATAMMC, Fort Belvoir, VA 22060.

1.7.2 Recognized Federal holidays: contractor is not required to perform services on the following holidays.

Note: Any of the above holidays falling on a Saturday will be observed on the preceding Friday; holidays falling on a Sunday will be observed on the following Monday. Any holiday that are declared by Presidential Executive Order shall be observed in the same manner as the holidays listed above. If the area in which contractor personnel are scheduled to work is closed due to the holiday and the personnel are NOT required to report in, payment will NOT be made for those hours. Closures of the installation due to inclement weather or other such acts of God shall be handled in the same manner.

1.7.3 Hours of operation: ATAMMC, the contractor shall provide personnel for contract support services from Monday to Friday between the hours of 0730 to 1600. Workdays will include a 30-minute unpaid lunch period. Every effort should be made to schedule a 30-minute lunch break between the hours of 1100 and 1300.

1.7.3 1.Contractor personnel working under this contract shall be paid only for services rendered. Contractor personnel shall coordinate lunch periods with the site supervisor. Payment to the contractor will not be made for temporary work stoppages due to circumstances beyond the control of ATAMMC such as acts of God, inclement weather, power outages, or temporary closing of facilities.

1.7.3.2 The contractor personnel will work no more than 80 hours in a two-week period under this contract. Contractor personnel will work no more than 12 hours in a 24-hour period.

1.7.3.3 The Contractor shall be responsible for a record of arrival and departure times, absenteeism, payroll records, coordinating time off with the Government, and appraisals of their personnel. The Contractor shall accomplish this requirement without requiring Government employees to engage in the contractor’s corporate operational or human resource management processes. The Government will not sign any contractor documents. The government reserve the right to verify hours worked by the contractor personnel by implementing a sign-in and sign-out procedure or by using a timeclock at a site other than the assigned ward/unit. Daily sign-in log, each day that services are performed, the contractor personnel shall be required to sign-in at the time of arrival and departure on a log sheet to be maintained within the department.

1.7.3.4 The contractor shall ensure notification of any unplanned personnel absences due to emergencies by notifying Government Site Supervisor, the Contracting Officer’s Representatives (COR), and the Vendor no later than two hours prior to the scheduled shift. Notification can be via email, phone call or text, IAW the unit’s protocol for call-outs.

1.7.3.5 The contractor shall ensure coordination of any planned personnel absences. The contractor will notify the Government Site Supervisor, COR or designee no later than 14 calendar days prior to the planned absence. Scheduled absences shall be limited to two (2) weeks. Planned absences will be IAW the unit’s protocol for requesting leave.

1.7.3.6 Overtime is NOT authorized

1.7.3.7 Emergency Services: On occasion, services may be required to support an activation or exercise of contingency plans during normal duty hours and may extend over if the need arises.

EXCEPTIONS: The Contractor and the Contractor's employee shall agree to extend these hours as necessary to ensure completeness of emergency care in progress prior to the close of the contractor employee's duty hours. In the event the contract HCW is required to work beyond the normal duty day, the duty hours will be adjusted within the subsequent five days of performance by mutual agreement between the contractor and the COR.

1.7.3.8 Pre-screening requirements:

1.7.3.9 No later than seven (7) days after notification of award, the contractor shall submit a complete Qualification Packages for each potential contractor personnel to the COR for approval.

1.7.3.10 Qualification Package: The packet shall consist of a resume, physical examination certificate, proof of required vaccinations, tuberculosis screening, diploma/degree, and any specialty job-related documents or certification as required. It is the contractor’s responsibility to ensure the candidates have all the requirements for this contract. Packets may be conditionally approved, pending receipt of remaining medical documentation prior to the start of work; however they must contain the following required documentation prior to employment; TB and drug test results, Basic Life Support training card, and Certification. No contractor personnel will be scheduled to work with an expired Basic Life Support card/certification. The contractor is responsible to backfill all scheduled shifts for all contracted personnel providing services under this contract that are unable to work due to expired training or certification.

1.7.4. Orientation: Contractor personnel shall complete all required Government orientation programs, initial and annual training.

1.7.5 Training: The Contractor will ensure all required training requirements are completed. The government reserves the right to add to and take away training requirements as deemed necessary to ensure proper execution of the services to be provided by the contractor personnel within the scope of this contract. The contractor may request additional support from the Government for training when resources are available. However, it is ultimately the contractors responsibility to ensure all training is conducted to a high standard consistent with executing the duties and responsibilities within this contract.

1.7.6 Prior to being assigned duties within this contract, contract personnel must ensure all training is completed and reported to the Program Manager for quarterly reports. Additionally, the Program Manager will be responsible for onboarding training to ensure familiarization with the duties and responsibilities of the personnel and to ensure safety at all times.

1.7.7 Government-Contractor Relationship: Contractor and Government personnel shall comply with all DHA policies referenced in the DHA guide for Government-Contractor relationships.

1.7.7.1 Contractor personnel shall complete all MTF and installation in-processing requirements. When a contractors personnel’s performance under this contract ceases, the contractor personnel shall return the Common Access Card (CAC), and all other government issued badges, per MTF policy and procedure.

1.7.8 Contractor personnel at ATAMMC and its outlying clinics shall meet the standards set forth in the ATAMMC Professional Standards Guide. Electronic copies of this guide is available on the Copies of this guide will be provided during in processing.

1.7.9 Conduct: Contractor personnel shall adhere to standards of conduct as established by the MTF.

1.7.9.1 Removal: At any time during the performance of this contract, the CO may direct the contractor to immediately remove any contractor personnel whose actions are not compliant with MTF standards and fail to comply with the MTF’s rules and regulations, whose abusive language, comments or impaired state raises reasonable suspicion that clear and present danger of physical harm exists to a patient, other contractor personnel, government personnel, or to the impaired individual. This provision will be used in emergency situations only and not for the purpose of bringing performance issues or other non-urgent concerns to the attention of the contractor. The contractor shall replace any contractor personnel serving on this contract that had to be removed from the workplace, within 15 working days or as agreed upon in writing with the CO.

1.7.9.2 If the need for a removal occurs, the CO will contact the contractor and direct the removal of that personnel from the facility and not use that individual to perform any services required under this contract until the issue has been resolved. The contractor shall formally meet with the COR to discuss further action. A review of the basis for removal will be made by the CO within three (3) working days after the CO directed the removal.

1.7.9.3 If, after any investigation deemed necessary by the CO and discussions with the contractor's representative, the CO concludes that the contractor personnel’s impairment requires permanent removal from performance under the contract, the CO will notify the contractor that permanent removal is required. In the event of disagreements between the government and the contractor's representative concerning matters of impaired contractor personnel, the decision of the CO will be final. During the period of time between the removal and the final decision of the CO, the contractor shall provide a backup or replacement contractor personnel in accordance with the terms and conditions of this contract.

1.8 Contractor travel: Reserved

1.10 Quality

1.10.1 Quality Control (QC): The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor’s quality control program is the means by which the work complies with stated requirements. The Quality Control Plan (QCP) shall be incorporated in the contractor’s proposal. A comprehensive written QCP shall be submitted to the KO and COR within 5 working days when changes are made thereafter. After acceptance of the Quality Control Plan (QCP), the contractor shall receive the contracting officer’s (KO) acceptance in writing of any proposed change to his QC system. See Technical Exhibit 1 - Contract Data Requirements List (CDRL) A001.

1.10.2 Quality assurance (QA): The government will evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan (QASP). This plan provides a systematic method for the Government to evaluate performance and to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s).

The designated Contracting Officer's Representative (COR) will record all surveillance observations and perform inspections as required on an unscheduled basis. When an observation indicates defective performance, the COR will require the Contract Manager or designated alternate to initial paperwork recording the observation. The initialing of the paperwork recording the observation by the Contract Manager or designated alternate does not necessarily constitute concurrence with the observation, only acknowledgment of the observation. The observation paperwork will be forwarded to the Contracting Officer for submission to the Contractor. The contractor will be required to correct the deficiencies or defective performance within 5 working days.

1.11 Contractor personnel

1.11.1 CAC requirements: See Seciruty Investigations Requirements in Part 6.

1.11.1.1 The contractor shall return all CACs to the COR upon the departure of the contractor(s).

1.11.2 Contractor onboarding and training. The contractor shall complete all requirements, training, and forms as prescribed in the following requirements:

1.11.2.1 ATAMMC on-boarding/off-boarding requirements are given to contractor during on-boarding/on-boarding scheduled date. All on-boarding instructions are sent to contractor once on-boarding date has been scheduled.

1.11.2.2 The DHA’s contractor training instructions embedded at Part 7 Section 7.1.2.

1.11.2.3 The contractor shall comply with onboarding requirements of the DHA for contractors needing to be issued CAC identification, including DoD- and DHA-directed training and forms submission, prior to network access, as displayed in the In/Out-Processing Portal at: https://info.health.mil/cos/admin/hr/IO/SitePages/home.aspx (note: Public Key Infrastructure (PKI)-restricted, printed versions available).

1.11.2.4 The DHA’s new employee handbook at Part 7 Section 7.1.4.

1.11.2.4.5 The Contractor shall prohibit the use of Government issued keys and key cards by any persons other than the Contractor personnel. The Contractor shall prohibit the opening of locked areas by Contractor personnel to permit entrance of persons other than Contractor personnel engaged in the performance of assigned work in those areas, or personnel authorized entrance by the CO.

1.11.2.4.6 Cybersecurity Requirements: All contractor personnel requiring access to a government information system shall successfully complete the Department of Defense (DoD) Cyber Awareness Challenge prior to access to the information technology, and annually thereafter.

1.11.3 PHYSICAL Security: The contractor shall be responsible for safeguarding all government equipment, information and property provided for contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured.

1.11.4 Key control: Reserved

1.14 Reporting

1.14.1 Contractor Manpower Reporting (CMR): Reserved.

1.14.2 Non-Disclosure Agreement (NDA): All contractor personnel who will obtain access to proprietary, classified, or confidential information or any information release of which is protected or governed by law or regulation associated with DHA acquisitions shall be required to complete and sign a DHA contractor NDA (DHA Form 49) prior to beginning work on the subject contract. The contractor shall execute an NDA on behalf of the company and shall ensure that all staff assigned to, including all subcontractors and consultants, or other personnel performing on contract/Task order execute an NDA protecting the procurement sensitive information of the Government and the proprietary information of other contractors. The NDA shall be executed not later than first day of employment and to be renewed upon exercising a contract option period. Assignment of staff who has not executed this statement or failure to adhere to this statement shall constitute default on the part of the contractor. The contractor shall maintain originally signed NDAs of individual employees and provide copy to the COR.

1.14.3 Government’s COR: The COR monitors all technical aspects of the contract and assists in contract administration. The COR is authorized to perform the following functions: assure that the contractor performs the technical requirements of the contract; perform inspections necessary in connection with contract performance; maintain written and oral communications with the contractor concerning technical aspects of the contract; issue written interpretations of technical requirements, including Government drawings, designs, specifications; monitor contractor's performance and notifies both the CO and contractor of any deficiencies; coordinate availability of government furnished property; and provide site entry of contractor personnel. A letter of designation issued to the COR, a copy of which is sent to the contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting contract.

1.14.4 Post award conference/periodic progress meetings: The contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with FAR Subpart 42.5. The CO, COR, and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings, the CO will apprise the contractor of how the government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government.

1.15 Absences due to an emergency: See 1.7.3.4

1.15.1 Contractor personnel performing services in a contractor capacity in a Government facility are required to possess and wear an identification badge that displays his or her name and the name of their company. All contractor personnel shall identify themselves as contractor support personnel in all forms of communication with all entities with whom DHA/Deputy Assistant Director for Acquisition (DAD-A)/Head of the Contracting Activity (HCA) has business dealings. The contractor shall: Answer all telephone calls and have a personalized voice message with an introductory statement that includes the fact that the person is contractor support personnel. Ensure all those with whom the person interacts in any face-to-face dealings while supporting the DAD-A understands that the person is contractor support personnel. Include a title block in all emails that states the fact that the person is contractor support personnel. Ensure all those with whom the person interacts in any face-to-face dealings while supporting DHA/DAD-A/HCA understands that the person is contractor support personnel.

1.15.2 Smoking policies: The contractor shall ensure that all contractor personnel comply with the facility smoking policies while performing services under this contract. Smoking restrictions may apply at any location or building where activities are performed under this contract.

1.15.3 Health and Immunization Requirements:

The Contractor shall ensure all contractor employees comply with all health requirements. No earlier than 30 days prior to physical performance of services by the Contractor, the Contractor shall provide to the Government documentation certifying its employees meet all health requirements such as immunizations, annual vaccinations, and medical testing (i.e., tuberculosis, N95 particulate respirator and duckbill mask fitting, Hep A (Food Service). (Attachment, Immunization Form).

Annual Immunizations: The Contractor shall ensure all contractor employees under this contract are immunized annually with the seasonal influenza vaccine and any other vaccine recommended by the Advisory Committee on Immunization Practices (ACIP) of the Centers for Disease Control (CDC) for contractors or service/ATAMMC specific guidance as outlined in DoDD 6205.02e, Policy and Program for Immunizations to Protect the Health of Service Members and Military Beneficiaries. If a contractor employee declines vaccination, the Contractor shall provide a signed declination form to the COR IAW CDC recommendations and ATAMMC policy.

Immunization Tracking: The Contractor shall maintain its own process and system of tracking the currency of health immunizations and shall not rely on the Government for ensuring its contractor employees are in compliance.

Blood Borne Pathogen Exposure: All contract providers/technicians performing health care services shall participate in the ATAMMC’s Blood Borne Pathogen Program, which outlines the post blood borne exposure protocols for testing, treatment, and monitoring.

Occupational Health: If required by the government, the Contractor shall obtain, at contractor expense, documentation of required physical testing, and a statement from the Contractor’s licensed medical practitioner or a report of a physical examination. The physical examination shall indicate that the Contractor is free from mental or physical impairments that would restrict the Contractor from providing the services described in the contract.

Universal Precautions: The Contractor shall comply with the CDC’s “Universal Precautions” for the prevention of the transmission of the human immunodeficiency virus (HIV). HIV positive contractor employees will be managed IAW the current CDC guidelines and Section 503 of the Rehabilitation Act (29 U.S.C. Section 793) and any implementing regulations (41 CFR § 60-741).

Health Notifications: The Government will notify the Contractor of any known work hazards. If work hazards exist, it will be the Government’s decision whether the Contractor continues work in the environment.

Medical Tests: No medical tests or procedures required by the contract will be performed by the Government with the exception post blood borne exposure protocols after exposure; and occupational exposure programs such as the Thermoluminescent Dosimetry monitoring program.

Failure to meet the requirements stated herein, or when test results determine a contract provider has a contagious disease, the Contracting Officer may, upon the advice of the ATAMMC commander or his clinical staff, determine that such provider is not an acceptable individual to perform services under this contract.

The contractor is responsible for maintaining the health records of its employees performing on this contract and making them available for inspection by the Government upon request.

Contractors performing services shall comply with the health and immunization requirements of ATAMMC, at the time of initial request for clinical privileges or employment and annually thereafter. Backup/replacement contractor employees shall be required to provide equally current certification of health at the time of initial request for clinical privileges or employment, and annually thereafter. The expense for all physical examinations and immunizations to comply with the health requirements shall be borne by the contractor at no additional cost to the Government.

Tuberculosis Screening: Prior to reporting for service at ATAMMC, the contractor employees shall be screened at Contractor expense for risk of exposure to tuberculosis as part of the health examination and immunization/screening requirement outlined in the contract. If the contractor employee is determined to have a low risk of exposure, no further screening or testing is required under this contract. The initial screening may be waived, at the discretion of ATAMMC, if the contractor employee provides evidence of a prior low risk assessment by a licensed physician. If the initial screening results in a determination that the Contractor has an increased risk of exposure to tuberculosis, the Contractor shall ensure that the contractor employee receives targeted screening and testing IAW CDC Guidelines for Health-Care Settings at Contractor expense and submitting timely records of subsequent screening or testing to the COR.

1.16 Contractor Access to ATAMMC Network(s)

1.16.1 FSO/Company's Security POC shall notify the DHA Personnel Security Office after being awarded a contract that requires access to a DoW system (If applicable, if not delete 1.16.1, 1.16.2, and replace to 1.16 Reserved). Contractor personnel requiring access to the HA/DHA networks for performance of their tasks require a background investigation and the security awareness training. The contractor shall be prepared for this process as it could take two (2) or more weeks.

The following documents are required to initiate an investigation.

· PSO Prospective Employee Profile Sheet (incomplete and/or illegible documents will NOT be accepted)

· OF-306 Declaration for Federal Employment-typed (original signature on the 2nd page; no copies will be accepted

· Two original Fingerprint Cards taken on FD-258 fingerprint forms. ATAMMC: Electronic Fingerprints also accepted if captured at an approved location on Secure Web Fingerprint Transmissions (SWFT).

· Copy of Driver’s License (Real ID) or Government issued ID Card.

· If born outside the U.S., proof of citizenship (i.e., copy of U.S. Passport, Naturalization Certificate, or Citizen Certificate) Dual Citizenship: When a candidate has conditions that could raise security concern and may be disqualifying; the COR responsible for the contract must schedule a meeting to have the candidate meet with the Security Manager to address the conditions prior to being deemed an acceptable risk for employment and/or in- processing. The candidate is not vetted and will require a representative to escort him/her while here at the ATAMMC. The candidate will be required to bring in original documents that show proof of U.S. Citizenship, any foreign passport in his/her possession (current or expired), and the U.S. passport when meeting with our office. The candidate will be given the following three options:

a) Voluntarily relinquish his/her foreign passport and express the willingness to renounce dual citizenship as a prerequisite in determining suitability and eligibility for a public trust or sensitive position.

b) Voluntarily relinquish his/her foreign passport and express the willingness to renounce dual citizenship directly to their Embassy or Consulate; and will provide ATAMMC: PSO proof of surrender of foreign passport on Embassy Letterhead. (Applicants understand that they may incur a financial cost to process at Embassy).

c) Refuse to relinquish his/her foreign passport and/or dual citizenship; understanding that NO access to the Network will be approved. By selecting this option, the member will not be approved to in-process.

Fingerprints: HCWs shall be subject to the following security processes for fingerprints: ATAMMC: Two original Fingerprint Cards taken on FD-258 fingerprint forms. Electronic Fingerprints also accepted if captured at an approved location on Secure Web Fingerprint Transmissions (SWFT).

SF-85/SF-86

· Once the candidate completes the questionnaire, he/she will be asked to certify the application and release the application to the security office (the security manager is not able to access the application for review until it is released). Before submitting the questionnaire, the candidate should make a copy for their records.

· The review of all documentation including the results of the fingerprints given by OPM will be conducted by the security office.

· Once the review is completed an email will be sent with the approval/disapproval of an interim while the investigation is ongoing to the COR.

1.16.2 Company’s FSO/Security POC must notify the COR when the contractor has submitted the SF-85/86. Any discrepancies with the SF-85/86 will be communicated from ATAMMC’s security office directly to the candidate and the COR. The candidate has the responsibility to comply with the instructions from ATAMMC’s Security Office.

The intiaition of the background check is sent from ATAMMC’s Personnel Security Office/eApp directly to the candidate and contains the direct number and email to ATAMMC’s assigned Security Officer/Designee.

The contractors FSO/Security POC must nofity the COR in writing of a contractor's termination from the contract, including the termination date. The COR will initiate out-processing procedures for ATAMMC.

1.17 Personnel Security

1.17.1 The contractor shall comply with DoD 8570.01-M, “Information Assurance Workforce Improvement Program, CH4” November 10, 2015 as amended; 8500.01, “Cybersecurity”, dated March 14, 2014; DoD Manual (DoDM) 6025.18, “Implementation of the Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule Compliance in DoD Health Care Programs” dated March 3, 2019, Department of Defense Instruction (DoDI) 6025.18 “HIPAA Privacy Rule Compliance in DoD Health Care Programs”, dated March 13, 2019; and DoDM 5200.02 “Procedures for the DoD Personnel Security Program (PSP),” incorporation change 3, effective September 24, 2020. Contractor responsibilities for ensuring personnel security include, but are not limited to, meeting the following requirements:

1.17.1.1 Follow the DHA Personnel Security Office guidelines for submittal of security clearances. Contact the DHA Personnel Security Office for guidance on the appropriate background investigation required for personnel on the contract. The DHA Personnel Security Office can be reached at (703) 275-6038.

1.17.1.2 Initiate, maintain, and document personnel security investigations appropriate to the individual’s responsibilities and required access to Controlled Unclassified Information (CUI).

1.17.1.3 DHA Personnel Security Office does not deny any access to any automated information system (AIS), network, or Controlled Unclassified Information (CUI). If a contractor receives an unfavorable background investigation, the request for access will be sent back to the FSO for further action. Any unfavorable adjudication will result in ATAMMC Personnel Security Office revoking any interim clearances and accesses.

PART 2

2.0 DEFINITIONS, ACRONYMS, AND APPLICABLE PUBLICATIONS/INSTRUCTIONS

2.1 Definitions:

2.1.1 Category D: Information Technology (IT) and Telecommunications Services (called D-Services)

2.1.2 Category R: Support (Professional/Administrative/Management) Services (called R-Services)

2.1.3 Contracting Officer (CO): A person with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings.

2.1.4 Contracting Officer’s Representative (COR): An individual, including a contracting officer’s technical representative (COTR), designated and authorized in writing by the CO to perform specific technical or administrative functions. This individual does NOT have authority to change the terms and conditions of the contract.

2.1.5 Nonpersonal services contract: a contract under which the personnel rendering the services are not subject, either by the contract’s terms or by the manner of its administration, to the supervision and control usually prevailing in relationships between the Government and its employees.

2.1.6 Quality Assurance Surveillance Plan (QASP): An organized written document specifying the surveillance methodology to be used for surveillance of contractor performance. The Government may either prepare the QASP or require the offerors to submit a proposed quality assurance surveillance plan for the Government’s consideration in development of the Government’s plan.

3.0 GOVERNMENT FURNISHED PROPERTY, EQUIPMENT, AND SERVICES

The Requiring Activity Authority has assessed the need for Government Furnished Property, Equipment, and Services and determined:

3.1 Services: The Government:

☒ WILL provide Government Furnished Services required in support of this contract/task orders. These Services are described below:

3.1.1 Government Training / Orientation - In addition to the specified courses listed below, Contractor employees are required to attend or complete on-line any Department of Defense, Defense Health Agency, Army, or Federal Government directed courses. Contractor employees are required to participate in all mandatory training.

3.1.2 Staff Orientation - New Employee. Contractor employees shall participate in all staff orientation and/or training. Contractor employees may need to attend a government-sponsored initial orientation to familiarize them with policies and procedures. Orientation attendance will be scheduled by the government supervisor. Such orientation may include instruction on automated processing, standard operating procedures, local in-services, quality improvement policies, communications, and occupational exposure to blood borne pathogens, safety programs, etc.

3.1.3 Annual Training Updates. Contractor employees may be required to complete annual training.

3.1.4 HIPAA Privacy and Security Training (HIPAA 101). Contractor may be required to complete the On-Line Web based Training Modules within the first 30 days of performance.

3.1.5 In addition to the specified courses listed above, Contractor employees are required to attend or complete on-line any Department of Defense, Army, or Federal Government directed courses. Contractor employees are required to participate in all mandatory training.

3.1.6 Government unique training. The government may elect to provide unique government training to Contractors who are performing services under this contract. If the government elects to provide such training, the government will provide such training at no additional expense to the contract employee or to the Contractor. When directed by the contracting officer, Contractors shall attend all such training in a paid status as part of the normal services required and billed under the contract.

3.1.7 Hours for attending any of the above shall be compensated at the regular hourly rate established in the contract.

3.2 Facilities: The Government:

☐ Will NOT provide Facilities in support of this contract/task order. As a result, this paragraph is Not Applicable.

☒ WILL provide Facilities in support of this contract/task orders. The Government provided Facilities are described below: The Government will provide the use of available DHA facilities and support services, desks, materials, publications and forms, and equipment required for contract performance (except as designated). Contractor shall keep Government furnished supplies, equipment and work areas in a safe, orderly and clean condition. Contractor shall notify the Government whenever maintenance of equipment is required. Contractor shall abide by all requirements for physical security of property and equipment. Space used by Contractor employees in performance of services may be used for other purposes during their absence. Items of clothing, personal effects, or equipment cannot be secured during their absence. The Government will not incur any liability for theft, damage to, or loss of such personal items.

Parking is not guaranteed to be available.

3.3 Utilities: The Government:

☐ Will NOT provide Utilities in support of this contract/task order. As a result, this paragraph is Not Applicable.

☒ WILL provide Utilities in support of this contract/task orders. The Government provided Utilities are described below: The Government will provide all utilities available in the facility. The Contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions that preclude the waste of utilities, which include turning off the water faucets or valves after using the required amount to accomplish cleaning vehicles and equipment.

3.4 Equipment: The Government:

☐ Will NOT provide Equipment in support of this contract/task order. As a result, this paragraph is Not Applicable.

☒ WILL provide Equipment in support of this contract/task orders. The Government provided Equipment is described below: The Government will provide telephones, facsimile machines, copiers and computer equipment to include laptops for use in performance under this contract/task order. This equipment is authorized for transaction of official Government business only and shall not be used for personal business. Personal long distance calls are not authorized and the cost of all personal long distance calls made by contractor or subcontractor employees may be deducted from the contractor/s invoice payments. Telephones, facsimile machines and computer equipment to include laptops are subject to communications security monitoring at all times. Items issued will remain the property of the Government and the contractor will maintain proper accountability of issued equipment. Equipment shall not be removed from the facilities shown in paragraph 3.2 above, unless otherwise specified in the PWS. They are to be used, turned in and/or disposed of as directed by the COR or CO.

Protective Clothing. The MTF will supply special protective clothing and shoe covers when required.

3.4.1 Procurement Integrated Enterprise (PIEE), GFP Module Application: N/A

3.5 Materials: The Government:

☐ Will NOT provide Materials in support of this contract/task order. As a result, this paragraph is Not Applicable.

☒ IS providing Materials in support of this contract/task orders. The Government-provided Materials are described below: Contractor employees may be issued keys. The Contractor shall safeguard the keys from loss, theft or destruction, and must display all keys signed for at scheduled or unscheduled key control inspections. The Contractor shall be required to reimburse the Government for lost keys, or lockset (if locksets are required to be replaced) as a result of lost keys. The cost of replacement of keys/locksets may be deducted from payments to the Contractor.

3.5.1 Materials (Administrative): The Government will provide Standard Operating Procedures and Policies.

3.5.2 Nametag. The Contractor health care provider shall wear a nametag furnished by the MTF. The nametag will be in compliance with and in accordance with MTF policy.

PART 4

4.0 CONTRACTOR FURNISHED ITEMS AND SERVICES

4.1 Services: The Contractor:

☐ Will NOT provide Contractor Furnished Services in support of this contract/task order. As a result, this paragraph is Not Applicable.

☒ WILL provide Contractor Furnished Services required in support of this contract/task orders. These Services are described below: provide all personnel and services to comply with the requirements of this contract, except where specifically identified as government-furnished supplies and services.

4.2 General: Reserved

4.3 Secret Facility Clearance: N/A

4.4 Materials: The Contractor will be responsible for all material unable to be provided by the government outlined in Part 3, to ensure proper execution of the duties and responsibilities within this contract. The contractor will furnish the uniforms outlined in 4.6 below.

4.4.1 The Contractor shall submit reports to the COR and/or the Contracting Officer as requested for use in monitoring Contractor performance. Such reports may include, but are not limited to, equipment inventory and maintenance reports, current license status, initial and/or monthly/annually immunizations, contract employee status.

4.6 Facilities: Reserved

4.7 Uniforms: (not applicable)

4.8 Contractor Identification: All contractor personnel shall be clearly identified as such at all times, including conversations, mail, e-mail, fax, and/or other electronic communication whether communicating with Government personnel, other contractor personnel, or with the public when supporting this contract. At a minimum, contractor management staff and contractor personnel shall clearly identify themselves as contractors by (1) wearing badges that clearly and legibly identify themselves as contractors, (2) using the label “Contractor” in e-mail addresses IAW Federal Acquisition Regulation (FAR) 37.114, Special Acquisition Requirements, (3) opening all phone conversations with a statement materially similar to, “[office name], I am [employee name], a [prime contractor’s company name] employee,” including the employing contractor’s name in the letterhead and/or signature block of any written correspondence, and (4) any other means necessary.

PART 5

5.0 SPECIFIC TASKS

5.1.1 Summary: Contractor shall provide Contract Support Services for Alexander T. Augusta Military Medical Center (ATAMMC), Fort Belvoir, VA.

All contract employees provided by the contractor shall meet the qualifications for that specialty and perform essentially the same functions, within the scope that is acceptable for their technical professional discipline and standard, as those required by DOD or government service technical professionals of similar experience and in similar duty assignments. Contract employee practices/productivity will be compared to that of other Contract employees assigned to the same medical center.

The Government has the right to test the contract employee the first day of the contract and any time during contract performance to determine whether the contract employee can perform all contract-required skills. If the contract employee cannot perform all those skills, the Government activity may request that the contract employee be replaced with a contract employee that can perform all the contract-required skills.

5.1. JUNIOR CONTRACT ANALYST

5.2 QUALIFICATIONS:

1. A 4-year course of study leading to a bachelor's degree with a major in any field, that includes or is supplemented by at least 24 semester hours in any combination of the following fields: accounting, business, finance, law, contracts, purchasing, economics, industrial management, marketing, quantitative methods or organization.

2. Minimum 2 years of experience serving as a Federal government and/or contractor employee in the GS-1102 series or equivalent capacity as a contract specialist/administrator or procurement analyst.

3. Must be DAWIA Contracting Level I certified (graduate level education may be substituted for years of experience or DAWIA Contracting Level I certification).

4. Knowledge of the federal acquisition process.

5. Knowledge of contracting principles, laws regulation and procedures.

6. Knowledge of methods and techniques of fact-finding, analysis, and resolution of complex problems, and the ability to develop concrete action plans to solve problems.

7. Familiarity with price and cost analysis principles.

8. Shall be able to read, write and speak English.

9. Shall be a U.S. Citizen

5.2.1.Specific Duties/Tasks the Contract Employee Shall Perform:

1. Accomplish and deliver compliant pre- and post-award documentation and/or contractual documents in accordance with the Federal Acquisition Regulation (FAR), the Department of Defense Federal Acquisition Regulation Supplement (DFARS), DHA and Department of Defense (DoD) supplements, policy guidance directives and instructions.

2. Shall complete operational or consulting support in one or more areas of the acquisition life cycle or program/project functional area, to include: purchasing/procurement, source selection, training, research, business process improvement, policy development, financial analysis, automated systems development/analysis/ implementation, federal government project or program management, federal statutes and regulations, acquisition or contracting management.

3. Perform most work independently in a non-managerial role, exercising much latitude over steps to carry out tasking, prioritize workload and make recommendations on overall strategies.

4. Provide a monthly report on all information pertaining to acquisition and contracting actions for review and status updates.

5. Provide adequate support through requests made through ATAMMC Contract Support. A monthly report will be provided to the Service Chief of ATAMMC Contract Support and the COR on active and outstanding requirements.

6. Understand aspects of the DHA Procurement Customer.

7. Formulate Performance Work Statements and various documentation for Pre-Award Requirements Packages.

8. Shall meet with Division Directors and other personnel to ensure the suggested requirement is developed correctly for the approval of contracting.

9. Assist Department Directors and Contracting Office in guidelines and procedures towards completing a requirements package for healthcare and non-healthcare requirements at ATAMMC.

10. Generate and update all required documentation and templates utilized in the submission of a requirement to contracting.

11. Conduct training and educational briefings to ensure all personnel seeking contracting support understand procedures and policy.

12. Shall educate and train new/secondary duty Contracting Officer Representatives (COR) outside the Resources Management Department, ATAMMC in completing requirement package…
