STATEMENT OF WORK

FOR

PWM02388, Replace of Aging Security Light Poles with Lighting Fixtures at Tengan Booster, Chimu-wan Tank Farm #3, Okinawa, Japan

GENERAL: This Statement of Work supplements the General Requirements within the Guide and Technical Specifications of PWM02388, Replace of Aging Security Light Poles with Lighting Fixtures at Tengan Booster, Chimu-wan Tank Farm #3, Okinawa, Japan and more specifically defines the subject project. The contractor shall provide all engineering contract documents needed to issue a construction contract.

I. GENERAL SCOPE: The contractor shall verify all actual measurements and conduct field surveys and investigations, load evaluation and testing. The contractor shall also provide all labor, materials and equipment necessary to complete this project.

II. STATEMENT OF WORK:

A. REPAIR WORK (K); Work shall include but not limited to:

CIVIL

1. Excavate/dispose and backfill with compaction and prepare the ground to required bedding surface for new structure.

2. Replace existing asphalt pavement concrete structure with new including 50mm thick asphalt concrete course, 150mm thick sub base course, 100mm thick base course that will be affected by the installation of new underground secondary duct.

3. Restores existing concrete structure same as existing condition affected by construction.

4. Provide 75mm thick sand cushion all around secondary ducts.

5. Provide underground marking tape for metal reaction detector type for new underground secondary duct.

6. Provide the marker (pin and concrete type) to a linear change point of this project.

7. All unpaved areas within the grading limits and affected by the construction shall be covered with 3” thick topsoil and solid sodding.

8. Provide rope off/barricade at all open trenches in accordance with standard construction procedures, to include hazardous/warning signs and perform traffic control with watchman.

9. Perform traffic and pedestrian control with watchman as required at day time and construction areas shall be continuously illuminated during the hours of darkness.

10. Provide the steel plate across the board for safety access to existing parking lots & roadways.

11. Rock Excavation: If there are rock excavated with a size greater than 1/2 cubic meter in volume, a rock survey report & pictures shall be submitted that quantifying the total volume of rock excavated. The rock removal will be accomplished by a negotiated modification of the task order.

12. Provide steel guard posts for security lights concrete pole. Guard posts metal surfaces shall be hot dip zinc coated with an average weight of not less than 550g/m ². Galvanized post surfaces have been cleaned and shall be given one coat of primer paint and two-finish coat of yellow reflection paint.

ELECTRICAL

1. Replace existing circuit breakers for security light and pavilion with new NEMA 1 Encl.

2. Install new safety switch, NEMA 4X ENCL for pavilion.

3. Replace existing underground feeder system for security light with new.

4. Remove existing overhead feeder system and install new underground feeder system for security light and pavilion.

5. Install new electrical handhole, handhole cover with grounding system.

6. Install new LED lights that has a resistance to salt laden atmosphere in Okinawa, concrete poles, hot dip zinc coated steel arm, guard posts and grounding system.

7. Install new lighting control panel, NEMA 4X ENCL, lockable type with selector switch, “ON-Auto-Off”.

8. Conduct ACM & LBP sampling and testing.

III. GOVERNMENT FURNISHED MATERIALS (GFM): NA

IV. DESIGN REQUIREMENTS: The contractor shall be responsible to obtain As-built drawings from the government. The drawings information is for contractor reference only. The Government shall not be responsible for any interpretation of or conclusion drawn from the data or information from the contractor.

A. Design must conform, as applicable, to the latest UFC (Unified Facility Criteria), IBC (International Building Code), NFPA (National Fire Protection Association) regulations, NESC (National Electrical Safety Cord), IESNA (Illuminating Engineering Society of North America), OSHA (Occupational Safety & Health Administration) standards, UL (Underwriters Laboratories), TI (Technical Instructions), Unified Facilities Guide Specifications (UFGS), Industrial Ventilation, (UFAS) Uniform Federal Accessibility Standards, JEGS (Japan Environmental Governing Standards), JIS(Japanese Industrial Standards), JASS (Japanese Architectural Standard Specification), Military Construction Guide Specifications, Torii Standard and all applicable Act, policies, regulations and publications.

B. Actual field investigation and survey must be conducted to ascertain existing conditions (locations) and work scope. All obstructions and known utilities must be shown on the drawings. These must be considered when creating the design. The contractor should not assume the existing facility is built per regulation.

C. Project design must be coordinated with the using agency and other agencies involved in project reviews.

D. Design Analysis: developed to each design submittals.

1. Civil Requirements:

· Schematic Description of Site Conditions

· Schematic Technical references (codes, manuals, directives)

· Schematic Drainage Storm water routing

· Schematic Analysis of the existing systems and new work requirements

· Schematic Traffic volume and type

· Schematic ATFP Standoff Distance

2. Electrical Requirements:

· Schematic scope of new work to be performed

· Load calculation

· Voltage drop calculation

E. Design Schedule: Contractor shall attend design review meetings as scheduled.

Total Design performance time 119 calendar days after the issuance of NTP.

*If the revised 95% design document is not acceptable to the government due to design deficiencies of the contractor, the contractor must resubmit the re-revised 95% design document with the contractor’s own expense.

NOTE: The contractor shall submit As-Built drawings.

G. Construction Time: The contractor shall be allowed to work 0800 to 1700 hours, Monday through Friday, except the US Legal holidays.

H. The contractor shall describe the following instruction in the contract drawings:

The contractor shall closely coordinate with the User thru the Contracting Officer for the schedule of the works to avoid or minimize downtime and interruption of the mission and operation.

J. All material and construction shall meet or exceed US Government and Japanese Construction Standards.

K. The project design must use the International System of units (SI) of measurement. Drawing size to be provided shall be A1 size.

L. Contractor shall restore to the original condition all facilities damaged or temporarily removed during construction.

M. All exterior materials, equipment and the system shall be corrosion resistant, and be able to structurally withstand winds and earthquake load required in accordance with UFC 3-301-01. All exterior materials and materials to be used in wet, moisture location shall be corrosion, rust, and mold resistant.

N. After fabrication work, all metal materials shall be hot dip zinc coated in accordance with JIS H 8641.

O. Exterior and interior color selections shall be in conformance with Army color schemes and standards.

P. Exterior paint material (water based) should be over 10 year’s durability.

Q. The contractor shall provide any structural and electrical design analysis associated with the project performed.

R. All damages affected by the project shall be restored to match with existing adjacent condition.

S. Temporary barricade shall be provided by the contractor during the construction to prevent other occupancy area from construction dust and for security.

T. Paint new exposed surfaces of all pipes, fittings, brackets, supports, hangers, metal surfaces and other accessories.

U. Must follow “Torii Standard”. (Rule of Material Approval Submittal, CAD Design, Concrete, etc…)

V. Power outages shall be coordinate with DPW and submitted, in writing, to the COR at least 45 calendar days in advance.

W. All boxes and enclosures for outdoor installations shall be weatherproof stainless steel material.

X. Provide reinforced concrete encasement of new underground utility lines consists of generally leveling concrete, base course and formwork systems (Including anchoring and bracing to control upward and lateral movement as required).

V. Asbestos and Lead Base Paint (LBP)

A. The contractor shall be responsible to identify & test of any asbestos and LBP associated with the work performed in accordance with EPA, OSHA, JEGS and applicable regulations and specification and Army Environmental Protection Section 01560 (April 2022). The contractor’s asbestos and LBP inspector shall conduct survey for the project work area to identify homogeneous materials. The contractor shall submit the sampling plan with CD, obtain the government safety and environmental approval and coordinate the sampling schedule with the government prior to taking any samples.

B. If sampling test found positive with ACM & LBP abatement & disposal will be accomplished by a negotiated modification of the Task Order.

C. The manifest for the asbestos and LBP shall be submitted to the government immediately after disposal.

D. The contractor shall be provide ACM & LBP test result with information of ACM & LBP abatement for modification.

E. The contractor is required to conduct Toxicity Characteristics Leaching Procedure (TCLP) analysis of LBP for disposal criteria the TCLP test shall be included in the proposal and submit the report. If the LBP test results does not contain lead, TCLP test shall not be conducted. The amount proposed for TCLP test shall be returned to the Government. The manifest for the asbestos and LBP shall be submitted to the government immediately after disposal.

NOTE : If the LBP analysis report of less than 0.5 % of regulatory levels. The funds allocated on this CLIN item shall be returned to the government

F. Description of work:

The specifications for asbestos & LBP sampling plan (include sampling location pictures & after repair work pictures) & abatement shall be provided by the contractor.

Note-1: Make sure the area will not be disturbed during air sampling prior to asbestos/LBP work. The area should prohibit personal entering for 2 hours prior and during the sampling in order to established proper background for Asbestos/LBP work.

Note-2: Air sampling- The contractor hire the one independent Private Qualified Person (the PQP) to perform all required functions. PQP has successfully completed training in and is accredited and where required is certified as, a Building Inspector, Contractor/Supervisor Abatement Worker, and Asbestos Project Designer as described by 40 CFR 763 and has successfully completed the National Institute of Occupational Safety and Health (NIOSH) 582 course “Sampling and Evaluating Airborne Asbestos Dust” or equivalent.

G. All bulk asbestos & LBP sampling taken from the existing wall & other interior & exterior architectural & structural components shall be permanently repair by the contractor.

VI. DOCUMENTS REQUIRED: All documents required under this Contract, including but not limited to, safety hazard analysis plans, all correspondence of any nature whatsoever and all other written communications, documents, drawings, reports, etc of every description shall be written in English or translated in English before being provided to the Government by the Contractor. Delays encountered because a translated copy was not submitted shall be the Contractor’s expense.

A. AF Form 66, Material submittal Log: The contractor shall provide and responsible for AF Form 66, Material submittal Log.

B. Acetate cellulose long-life reproducible originals of all as-built drawing with CAD data are required before final payment, as required by FAR 52.236-21 Specifications and Drawings for Construction (APR 1984). Contractor shall also provide Windows compatible copies on disk of all drawings and documents in Adobe Acrobat (.pdf), Microstation (.dgn) and AutoCAD (.dwg) Format. Other software programs or systems or system formats will only be acceptable if approved by the government.

C. Require the KTR to submit AF Form 3064 and 3065. (14 calendar days after the acceptance of the final 95% design)

VII. GOVERNMENT FURNISHED UTILITY: Government Furnished Utilities will be provide where available at no cost to the contractor.

VIII. AVAILABILITY OF LATRINE: The contractor shall coordinate with the government for the availability of latrine. The contractor will be allowed to use the Bldg#306 or #307, Naha Port.

IX. FACILITY AVAILABILITY: Facility is available during the construction. The impact caused by this project shall be at minimum. The contractor shall coordinate the construction schedule with the government prior to the construction start. All rooms and the utility system except for the construction floor/phase shall be kept in useable condition during the construction. The contractor shall establish work procedures and methods to avoid interference with the business operations or adjacent to the construction area.

X. ENVIRONMENTAL REQUIREMENTS:

A. The contractor shall provide construction and demolition waste information including the costs to the Government immediately after the disposal.

a) Solid Waste manifests (Page A and E) shall be submitted to Environmental Office within 30 days or sooner of each disposal if solid waste disposal would be conducted for this project.

b) Hazardous Waste manifests (Page A and E) shall be submitted to Environmental Office within 30 days or sooner of each disposal if hazardous waste disposal would be conducted for this project.

B. For new constructions materials, ensure all are free of asbestos and lead-based paint (LBP) in accordance with the Japan Environmental Governing Standards (JEGS) or other laws and regulations. The contractor shall provide manufacture’s certification that materials contain no asbestos (≦ 0.1%) and no lead. If certification is not available, the contractor shall conduct sampling and analysis of the materials. The analysis results must show no asbestos and no lead.

XI. The contractor shall comply with the following requirement.

Energy Efficient Products:

52.223-15 – Energy Efficiency in Energy-Consuming Products (Applies to products delivered, used in the performance of the work, furnished for government use, or specified in the design of a building or work.)

XII. SAFETY REQUIREMENT:

A. The contractor shall provide and submit a site specific safety plan prior to start work for approval from the government. The contractor shall be responsible to obtain the approval in timely manner considering the government’s review time and the contractor’s review time.

B. The contract location is a multi-employer workplace. The contractor is required to provide information regarding hazardous materials being brought to this workplace and hazardous conditions produced by construction activities, and to provide information to the US Government how Human Health will be protected.

C. The contractor shall submit Safety Data Sheets and other information from contractor regarding protecting Human Health of US Government personnel. Since 1 June 2015, 29 CFR 1910.1200 (OSHA) requires that a Safety Data Sheet (SDS) be provided for each hazardous material used in the workplace.

D. Provide rope off/barricade at disturbed area in accordance with standard construction procedures, to include Japanese and English Bilingual hazardous/warning signs.

E. Perform traffic and pedestrian control with watchman as required at day time and provide lighting system at night time.

F. Provide the steel plate across the board for secure to safety access existing parking lots, roadways as required.

G. Provide scaffolding and shoring including, sheet piling to protect workmen, banks, adjacent paving, structures, in accordance with Corps of Engineers manual EM 385-1-1 as required.

XIII. FIRE SAFETY REGULATION:

The Contractor shall obtain a permit from the USAG-O Fire Chief prior to conducting any welding or soldering or whenever the use of open flame is necessary during the construction phase. The Contractor and his employees will be required to adhere to AR 420-1, dated 12 February 2008.

XIV. OPERATION SECURITY (OPSEC) AND SOCIAL NETWORKING:

A. The contractor personnel that will work under this contract shall review and sign the Contractor OPSEC Training Completion Verification Sheet within 30 calendar days after contract awarded. The DPW will provide the OPSEC Training slides to the contractor to take this training. Once the contractor’s training is completed the DPW shall file the signed Contractor OPSEC Training Completion Verification Sheet in the contract file.

B. Upon completion of contract, the contractor shall return any photos taken, along with all maps, sketched, other images to DPW.

C. The contractor shall not discuss and provide any information, documents, photos, maps, sketched and other images to any outside agency.

D. The contractor will not require access to US Government computers.

E. AT/OP SEC requirement

a) AT Level I training. This standard language is for contractor employees with an area of performance within an Army controlled installation, facility or area. All contractor employees, to include subcontractor employees, requiring access Army installations, facilities and controlled access areas shall complete AT Level I awareness training within 30 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable and annually thereafter. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the COR or to the contracting officer, if a COR is not assigned, within 05 calendar days after completion of training by all employees and subcontractor personnel. b) Access and general protection/security policy and procedures. This standard language is for contractor employees with an area of performance within Army controlled installation, facility, or area. Contractor and all associated sub-contractors employees shall provide all information required for background checks to meet installation access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce must comply with all personal identity verification requirements (FAR clause 52.204-9 or NAF Clause BI.142, Personal Identity Verification of Contractor Personnel) as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.

c) For contractors that do not require CAC, but require access to a DoD facility or installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05/AR 190-13), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.
