About this file
The document is a Public Contract Code Disclosures form for a government contract bidding process involving the State of California. The form requires bidders to complete three specific statements under penalty of perjury: Section 10285.1 Statement (regarding prior convictions related to public works contracts), Section 10162 Questionnaire (concerning disqualification from previous government projects), and Section 10232 Statement (addressing federal court findings of contempt related to National Labor Relations Board orders).
The disclosure form is designed to verify the bidder's legal and ethical standing in previous government contract work, with potential bidders required to declare any past violations, disqualifications, or legal issues within the preceding two to three years. The document emphasizes that providing false information could result in criminal prosecution, and bidders must carefully complete and sign each section, checking the appropriate boxes to indicate their compliance status. The form serves as a screening mechanism to ensure contractor integrity and accountability in public works contracts.
