About this file
This is an attachment to a federal contract that establishes security clearance and badging requirements for contractor personnel working on a clinical services contract at the Department of Health and Human Services (HHS) Field Operations Headquarters (FOH).
All contractor personnel (PerSCs) must obtain and maintain a Tier 2 MBI Security Clearance from FOH, which must be renewed every five years from adjudication. The contractor is responsible for submitting complete badging packages for all employees throughout the contract lifecycle. Required documentation includes the Information needed form (HHS, Sept. 2021), HHS-828 form, OF-306 Declaration for Federal Employment, current resume with complete mailing addresses, two non-digital fingerprint cards, Cybersecurity Awareness Training Certificate signature page, HHS Rules of Behavior signature page, and a Job Specific Position Description. The contractor must ensure package accuracy; incomplete submissions are rejected and resubmitted packages incur disincentives per Section C.11.2. Applicants must complete the OneView Gateway application within five calendar days of receiving submission instructions and respond to subsequent enrollment emails from HSPD-12 Services/HHSIdentity within 48 hours to schedule digital fingerprints and photos for FOH PIV badges. The security adjudication process may take up to 60 days; no personnel may access sensitive personally identifiable information, personal health information, or secure physical spaces prior to favorable adjudication. Applicants located over 60 miles from regional badging offices must submit fingerprint cards by mail. Renewal requirements and lost/stolen badge procedures are detailed, and penalties for non-compliance are assessed as invoice deductions. Additional requirements apply for IRS and Customs and Border Patrol personnel, requiring separate forms and processes.
