About this file
The Proposed Subcontractor Form is a procurement document for a California state and local contract opportunity related to the Annenberg Community Beach House. The form is specifically designed to list subcontractors performing work exceeding 0.5% of the total bid, with detailed requirements for disclosure of subcontractor information including their portion of work, location, license number, license class, expiration date, phone number, and contract amount. Bidders are required to specify subcontractors for any significant work portions, and failure to do so means the prime contractor must perform that work with their own workforce.
The document emphasizes strict compliance with California Public Contracting Code sections 4100-4114, mandating that any substitution of subcontractors or subletting of work over 0.5% of the total bid requires written consent from the project owner. Bidders must provide accurate license information within 24 hours of bid opening, with potential bid disqualification for non-compliance. The form provides space for listing multiple subcontractors, highlighting the project's potential complexity and the need for detailed subcontractor management in this swimming pool chemicals and storage tank maintenance contract for the Annenberg Community Beach House.

PROPOSED SUBCONTRACTORS
Complete this form to list subcontractors for any portion of work to be performed under Contract in excess of one half (1/2) of one percent (1%) of the Bidder’s total bid and provide other requested information. Bidder is solely responsible for correcting any errors. Corrections to the license information shall be provided within 24 hours of the bid opening. Failure to submit corrected California Contractor’s license information as set forth above will cause the bid to be non-responsive. Make copies of this page, if needed to list subcontractors.
Pursuant to the provisions of Sections 4100 to 4114 inclusive, of the California Public Contracting Code, every bidder shall in his bid set forth the portion of work that will be done by each subcontractor. If the Bidder fails to specify a subcontractor for any portion of work to be performed under Contract in excess of one half (1/2) of one percent (1%) of the Bidder’s total bid, he agrees to perform that portion with his own workforce. The successful Bidder will not be allowed to do either of the following without the written consent of the Owner.
1. Substitute any person as subcontractor in place of the subcontractor designated in the original bid
2. Sublet or subcontract any portion of work in excess of on half (1/2) of one percent (1%) of the total bid as to which his original bid did not designate a subcontractor
