About this file
This document is a Proposal Letter and certification form from the Douglas County Board of Commissioners (Douglasville, GA) Purchasing Department. The document serves as a binding offer template where vendors must certify their understanding of and compliance with the County's specifications, confirm their pricing will remain valid for 60 days from the bid opening date, and agree to deliver goods/services that meet or exceed the stated specifications.
The document outlines certification requirements regarding collusion and fraud prevention, requiring bidders to certify that their offer is made without prior arrangements with other firms and complies with the Official Code of Georgia Annotated, Sections 45-10-20. The form requires an authorized signature, printed name, company information including address and email, and includes the Douglas County logo and purchasing department address. No specific pricing terms, set-asides, or funding requirements are detailed in this document as it appears to be a standard certification form that would accompany a full bid submission.
