About this file

This is a Project References Questionnaire for the City of Daytona Beach, Florida's Lift Station #69 Rehabilitation project (ITB 2633). The comprehensive scope involves demolition of the existing dry-pit station, rehabilitation of the wetwell, installation of two new 15-horsepower submersible pumps, construction of new electrical services including a 50-kilowatt diesel generator with automatic transfer switch, installation of level monitoring and control systems with a Radio Telemetry Unit, modifications to force mains, and site work including excavation, dewatering, backfill, paving, sodding, and landscaping. A non-mandatory pre-bid conference is scheduled for March 20, 2026, at 10:00 a.m. via Microsoft Teams. The deadline for questions is April 9, 2026, at 5:00 p.m., and all bids must be submitted online through the City's OpenGov Platform at https://procurement.opengov.com/portal/codb by April 16, 2026, at 2:00 p.m., with bid opening occurring the same day at 2:00 p.m.

Bidders must possess an active Florida General Contractor (CGC) license with a minimum of five years of successful operation in Florida and must complete this questionnaire documenting qualifications including descriptions of three similar completed projects and references from three municipalities. The questionnaire requires disclosure of years in business, relevant project experience, history of project completion, equipment ownership and rental plans, and anticipated site problems and solutions. Bid security of 10 percent is required, and performance and payment bonds equal to 100 percent of the contract price are mandated unless the contract price is less than $100,000. Insurance requirements include workers' compensation, commercial general liability with minimum limits of $1,000,000 per occurrence, and excess liability of $2,000,000 per occurrence. The contract will be subject to Florida's Prompt Payment Act with 5 percent retainage until 50 percent completion and 5 percent thereafter for contracts valued at $200,000 or more, and liquidated damages will apply for failure to meet schedule deadlines. The City will hold bids for up to 60 days prior to award.
