This is a Prohibition Against Contingent Fees affidavit required by the Flagler County Board of County Commissioners for the Tourism Website Design, Development & Maintenance contract opportunity in Florida. The document mandates that all proposal submissions include a notarized statement in which the responding firm certifies that it has not employed or retained any third party, except bona fide employees, to solicit or secure the contract agreement. The firm must warrant that no fees, commissions, percentages, gifts, or other consideration contingent upon or resulting from the award of the contract have been paid or agreed to be paid to any external company, person, corporation, or individual.
The affidavit must be signed by an authorized corporate officer and notarized before a notary public, with the notary attesting to the signatory's identity and affixing their official seal and commission information. This sworn statement is required in compliance with Florida Statute 287.055(6)(a) and serves as a certification of ethical business practices and the absence of improper inducements in the contracting process.
