About this file

The document is a Sworn Statement under Section 287.133(3)(a) and 287.135 of the Florida Statutes, pertaining to Public Entity Crimes and Scrutinized Companies. This form is a standard legal document that vendors must complete when participating in public contracts, specifically designed to certify the vendor's compliance with state regulations regarding criminal history and international business activities. The form requires the signatory to affirm that their company has not been convicted of a public entity crime, is not listed on scrutinized companies lists for activities in Sudan or Iran, and understands the legal implications of such listings, which could prevent bidding on or renewing contracts with governmental entities for goods or services valued at $1 million or more.

The document outlines detailed definitions of public entity crimes, convictions, and affiliations, requiring complete transparency about any past criminal activities related to business transactions with public entities. It specifically mandates disclosure of any convictions after July 1, 1989, involving fraud, theft, bribery, collusion, or material misrepresentation. The form must be signed in the presence of a notary public and includes provisions for both scenarios where a company has or has not been convicted of a public entity crime, with specific instructions for each circumstance. This document is a critical compliance requirement for businesses seeking to enter into contracts with Florida governmental agencies, ensuring ethical business practices and protecting public interests.
