About this file

This document is a Quote Sheet for Montana Occupational Health Medical Exams issued by the Montana Army National Guard (MTARNG), Solicitation Number W9124V25QA016. The solicitation seeks occupational health medical clinical services from a board-certified occupational medicine physician to conduct job-related medical surveillance screening examinations for MTARNG federal technicians potentially exposed to workplace health hazards. The medical exams will be performed in or within 50 miles of Helena, MT, and include comprehensive baseline medical examinations for different job categories: Surface Maintenance, Aircraft Maintenance, Allied Trades, Electronics, and a standalone Audiogram.

The pricing structure is a firm-fixed price contract, evaluated on best value considering price, past performance, and technical capabilities. Vendors must price each exam type and its individual test components, which include physical exams, blood tests, urinalysis, lead screening, vision tests, chest x-rays, and specialized tests like cadmium and chromium screening. The solicitation is not a small business set-aside, uses NAICS code 621111, and requires vendors to be registered in the System for Award Management (SAM). Quote submission deadline is August 14, 2025, at 10:00 a.m. MST, with questions accepted until August 11, 2025, via email to the contracting officer.
