About this file

This is a pricing template appendix that provides instructions for completing three distinct pricing submission formats related to pharmaceutical products in a federal contracting context.

The BPA section requires vendors to complete pricing tables for up to 10 National Drug Codes (NDCs), with pricing based on Package Size (column G) and Price per NDC (column F). The Price per Unit is automatically calculated using the formula: Price per NDC divided by Package Size. Vendors should only complete tables for their actual NDCs and may submit zero quotes for specific condition sets, which will be flagged in red to indicate non-participation. The ADP (non-FAMP Discount) section requires submission of non-FAMP prices and Current Annual Federal Ceiling Prices, with automatic calculation of Total Calculated Refund using the formula: ((non-FAMP-FCP) + (non-FAMP*Y%)). A 0.0000% refund submission highlights red to indicate non-participation. The ADP (WAC Discount) section applies only to products without a Federal Ceiling Price, such as diabetic supplies, and requires Wholesale Acquisition Cost pricing with a percentage discount calculation. All sections note that subsequent rows auto-populate after the first entry, all non-yellow cells are locked for editing, and vendors should reference Formulary Notes for P&T committee decision-making context. Vendors exceeding 10 NDCs should contact the Points of Contact on the RFQ document for an expanded template.
