About this file

This is a Weekly Payroll Records Report and Statement of Compliance form issued by the Massachusetts Department of Labor Standards (DLS) for public works projects governed by Massachusetts General Law Chapter 149, Section 27B. The document establishes mandatory payroll record-keeping and reporting requirements for all contractors and subcontractors working on public works projects. Contractors and subcontractors must maintain true and accurate records of all persons employed on the project and submit certified weekly payroll records to the awarding authority either by regular mail or email. These records must be submitted weekly and within 15 days after completion of the contractor's or subcontractor's portion of work. The payroll forms are available from the DLS at www.mass.gov/dols/pw and must include all information required by law. All records must be preserved for three years from the date of project completion, with the awarding authority responsible for maintaining collected records for the same period.

The Statement of Compliance form requires an authorized officer or supervisor of wage payments to certify that all mechanics, apprentices, teamsters, chauffeurs, and laborers employed on the project have been paid in accordance with wages determined under Massachusetts General Law Chapters 149, Sections 26 and 27. The signatory must identify themselves by name and title, specify the contractor or subcontractor entity, and identify the building or project. This compliance certification must accompany all submitted weekly payroll documentation. The form ensures wage and hour law compliance across public works projects and provides the awarding authority with documentation necessary for oversight and enforcement of prevailing wage requirements.
