Statement of Work (SOW)
STATEMENT OF WORK (SOW)
for
POTABLE WATER CROSS-
CONNECTION CONTROL AND
BACKFLOW PREVENTION SURVEY
Malmstrom Air Force Base
MONTANA
February 2025
DESCRIPTION OF SERVICE
1. SUMMARY OF EXPECTATIONS
The Contractor shall accomplish activities necessary to support the Malmstrom AFB Cross- Connection Control and Backflow Prevention Program. The primary goal of this contract is to assist the base in complying with the Air Force Manual (AFMAN) 32-1067, Civil Engineering Water and Fuel Systems (04 August 2020), requirement to conduct a cross-connection control and backflow prevention survey every five (5) years. Specific objectives are as follows:
1. Conduct a cross-connection control/backflow prevention device survey of all water-using facilities.
2. Update the cross-connection control/backflow prevention device inventory with data collected during the field survey (see attached Appendices).
3. Develop a cross-connection control/backflow prevention survey memorandum report that includes findings from the survey and prioritized recommendations for program improvement along with associated cost estimates.
1.1. The Contractor shall provide all labor, hardware, equipment, and incidentals necessary to perform a full cross-connection control survey of the potable water system at Malmstrom Air Force Base (MAFB), MT, both on the main base and at 15 remotely located Missile Alert Facilities (MAFs).
The Contractor shall inspect each building suspected of having a connection to the potable water system including a 100-foot radius surrounding the outside of each building surveyed. The survey team shall consist of at least one person trained and experienced in cross-connection survey methodology.
1.2. At a minimum the contractor will comply with the following governing regulations:
1.2.1. The survey will be completed in accordance with AFMAN 32-1067. It is the primary governing document for this work.
1.2.2. The Uniform Plumbing Code (UPC) and the UPC Illustrated Training Manual are adopted under AFMAN 32-1067, latest edition.
1.2.3. UFC 3-420-01, Plumbing Systems, latest edition.
1.2.4. International Plumbing Code (IPC), latest edition.
1.2.5. When there are conflicts between AFMAN 32-1067, UPC, and IPC backflow requirements, the documents take precedence in the order listed.
1.2.6. Local regulations may apply. When state and local regulations apply, the base must comply with the more stringent requirement.
1.3 The following non-testable devices are not included in the scope of work for this survey:
1.3.1 Hose bib vacuum breakers (low hazard)
1.3.2 Laboratory sink vacuum breakers
1.3.3 Atmospheric vacuum breakers
1.3.4 Dual check valve devices
1.3.5 Vented dual check valve devices
1.3.6 Other non-testable devices integral to food service, medical, and other commercial and industrial equipment
1.4 Individual evaluation of the following equipment shall not be included in the survey:
1.4.1 Laboratory and mop sinks
1.4.2 Toilets
1.4.3 Urinals
1.4.4 Low hazard food service and commercial equipment
1.4.5 Low hazard equipment with built-in backflow protection
1.4.6 Equipment drain lines
1.4.7 Domestic laundry equipment
1.5 The Base Bioenvironmental Engineer and 341 CES Backflow Program Manager (BPM) shall be notified of all high hazards when identified during the survey in order that any serious health threat can be corrected immediately.
1.6 At the completion of the survey the contractor will provide the 341st Civil Engineering Squadron (341 CES) with two paper copies of the report and supporting data, and one electronic copy via email or DoD SAFE.
2. HOURS OF OPERATION
2.1. Normal Hours.
Main Base: 0700 – 1600 Monday – Friday Missile Alert Facilities: 0645 – 1715 Monday – Thursday
The contractor shall perform all the survey services required under this contract during the following hours: 0700 through 1600 Monday through Friday, with the exception of Federal Holidays, or when the base is officially closed by the base commander.
2.2. Recognized Holidays. The contractor is not required to provide routine service on the following days: New Years Day, Martin Luther King Day, Presidents Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. NOTE: Normally, if the holiday falls on Saturday, it is observed on Friday. If the holiday falls on Sunday, it is observed on Monday.
3. SECURITY REQUIREMENTS.
3.1. Security Information
The contractor shall comply with pertinent Air Force security regulations while working on the project. Contractor personnel are required to abide by all real-world Force Protection Condition (FPCON) measures, random anti-terrorism measures (RAMs), shelter-in-place orders, evacuations and lock-down notices as directed. Examples include mandatory badge or ID checks, facility single points-of-entry, and searches of hand carried items.
3.2. Anti-Terrorism (AT) Awareness Training
In accordance with DoDI O-2000.16v1_AFI 10-245-O, Antiterrorism (AT) Program Implementation, ensure Level I - AT Awareness Training is completed by all contractors within 30 days of requiring access, and annually thereafter. The training will be provided by the government. The objective of the training is to provide personnel with requisite knowledge necessary to remain vigilant and enable employment of AT tactics, techniques, and procedures.
3.3. Main Base Entry
The Contractor shall furnish a Contracting Entry Authority List (CEAL), attached, of employees needing access on base to the Contracting Officer (CO). Employees without a badge and identification in their possession will be denied access to the base and work areas.
3.4. Missile Field Entry
In order to gain entry onto a missile site, Contractor personnel must be listed on an Entry Authorization List (EAL), identified by Government inspector/escort personnel, and physically escorted on site by authorized Government inspector/escort personnel. The number of personnel authorized entry shall be kept to a minimum. The general contractor shall submit all sub-contractors that need to be on the EAL, but must limit personnel to a reasonable number. The CO shall deny any EAL request that has excessive numbers of workers listed. Entry procedures listed below may change in accordance with increased threat levels without prior notice.
Thirty (30) working days prior to the commencement of work, the Contractor must submit to the CO for the Missile Engineering Scheduler a form showing the name EXACTLY as it appears on the state issued ID, birth date, Social Security Account Number (SSAN), state issued driver's license number and state from which it was issued for each worker. The CO will provide the form (Excel format) to the Contractor. This list will be used to prepare the EAL referenced above.
Note: Being listed on the MAFB CEAL does not automatically qualify you to work in the MAFB Missile Field.
Deletions to the EAL may be made without preparing a new list but must be requested by the Contractor in writing. However, a new list must be submitted by the Contractor for any additions to the EAL. All requested changes in writing to an approved list may require up to thirty (30) working days for approval.
4. EMPLOYEE CERTIFICATION
Contractor shall have, or employ personnel that have, appropriate training and experience in cross connection control surveys.
5. ENVIRONMENTAL, SAFETY, AND HEALTH
5.1. Comply with the highest degree of environmental protection or safety practices where any disagreements between requirements exist.
5.2. Contractor Liability. The contractor shall be liable and responsible for all costs resulting from non-compliance, whether accidental or intended.
5.2.1. Hazardous Chemical Discharge. Any discharge or spills resulting from contractor negligence.
5.2.2. Non-Compliance with Environmental Regulations. Failure to follow any Federal, State, or local laws or regulations.
5.3. Fines and Corrective Actions. Any fines or corrective actions assessed by any regulatory or governing agency due to the actions or omissions of the contractor in carrying out his duties.
6. MISHAP NOTIFICATION AND REPORTING.
6.1 The Contractor shall immediately report any and all injuries and damage involving contractor, Air Force civilian or military personnel to the QAE and the Wing Safety Office.
6.2. Cooperate fully and assist government personnel in any government investigation of the mishap, until released by the mishap investigative authority through the CO. Safety personnel shall have the right, at any time, to investigate and report all mishaps involving Air Force personnel and property. The CO and contractor Safety representative may accompany Air Force mishap investigation personnel but may not release information regarding the mishap without written approval of the AF Safety investigating official or senior installation commander.
6.3. Maintain records of contractor employee’s occupational injuries, illnesses, and other related activities as required.
7. FIRE PREVENTION. Establish and implement procedures to ensure careful handling and storage of flammable and combustible products IAW National Fire Protection Association (NFPA) Standard 30 and National Fire Codes.
8. HOUSEKEEPING AND CUSTODIAL SERVICES. The contractor must maintain the contractor assigned areas in a clean, safe, and sanitary condition. Remove trash from all the work areas and store all flammables in an approved flammable storage container and ensure the area is left in an orderly manner at the end of the duty day.
8.1. Sanitary Facilities. The Contractor’s employees may use the nearest sanitary facilities. The Contractor shall not use facilities in controlled or restricted areas IAW security requirements.
8.2. Refuse Collection. Waste generated by the Contractor in performance of the services specified with this SOW shall be disposed of by contractor off base in compliance with State and Local ordinances, including hazardous materials in which proper disposal instructions shall be complied with.
9. ADMINISTRATIVE RECORDS.
All records, documents, and associated papers provided by the government and generated during the period of this contract become government property and will remain in place upon contract termination or completion. During the period of the contract, records, documents, and associated papers shall be available for review.
10. INSPECTION.
10.1. Backflow Program Manager (BPM) The 341 CES BPM will verify accomplishment of work. Alternate Quality Assurance Evaluators (QAEs) may be designated by 341 CES to support the cross connection survey, especially pertaining to work at the MAFs.
The BPM will be the spokesman for compliance with base standards.
Controversies between the BPM and the Contractor will be resolved by the CO. If, for some reason, a change in the BPM is required, the Contractor will be notified in writing by the CO. The Contractor shall coordinate Contractor activities with 341 CES.
10.2. Final Inspection. The Contractor will notify the BPM of their estimated work completion date at least 3 days in advance, to verify accomplishment of work and receive the Contractor work report. Any deficiencies found shall be rectified by the Contractor and the Contractor shall inform the BPM upon completion.
