About this file
This is an application template document for Type 6 applications under the Centers for Disease Control and Prevention's Injury Prevention and Control Research and State and Community Based Programs cooperative agreement (CFDA 93.136, RFA-TP-18-000). The application deadline is September 30, 2030. Eligible applicants include U.S. state governments, territorial governments, local governments, and their bona fide agents, with only one application funded per state or territory. Applicants must have access to state or territory-wide data sources and demonstrate organizational capacity to implement proposed injury and violence prevention strategies across domains including child abuse prevention, sexual violence prevention, firearm-related violence reduction, and overdose prevention.
Financial assistance provides 12-month budget periods with a performance period of up to five years, contingent on fund availability and satisfactory recipient progress. Typical annual award amounts range from $100,000 to $700,000, with most projects targeting 3–5 year implementation periods. The review process occurs in two phases: Phase 1 assesses eligibility and completeness, while Phase 2 evaluates complete applications based on approach, evaluation and performance measurement capabilities, and organizational capacity. After initial awards, projects may continue non-competitively if recipients demonstrate satisfactory progress and continued federal interest.
