About this file

This file is an application submission document designated for Type 6 applications under RFA-EH-18-000. The CDC's Centers for Disease Control and Prevention, through CFDA 93.136 (Injury Prevention and Control Research and State and Community Based Programs), is soliciting cooperative agreement applications to develop, implement, and promote effective injury and violence prevention practices. The application deadline is September 30, 2030, with the opportunity posted October 20, 2023. Eligible applicants include U.S. state governments, territorial governments, local governments, their bona fide agents, universities, county governments, small businesses, and non-profit organizations; however, only one application per eligible state or territory will be funded. Typical annual award amounts range from $100,000 to $700,000, with recent opportunities targeting $350,000 to $700,000 annually. Financial assistance is provided for a 12-month initial budget period with a performance period of up to five years, contingent on fund availability and satisfactory recipient progress.

Applications undergo a two-phase review process. Phase I evaluates eligibility and completeness by the CDC Office of Grants Services, with non-responsive applications eliminated before Phase II. Phase II review assesses approach, evaluation and performance measurement, and organizational capacity to implement proposed strategies. Applicants must have access to state or territory-wide data sources and demonstrate potential for developing scalable prevention strategies. Research and non-research applications are reviewed on scientific/technical merit, considering clarity of purpose, personnel qualifications, feasibility of producing meaningful results, alignment with national health priorities, and budget reasonableness. Recipients will be expected to work with national organizations, state health agencies, and key groups to implement injury and violence prevention strategies, with performance location at the recipient's site and potential nationwide implementation through local partnerships.
