About this file
This file is an application submission document for a CDC Cooperative Agreement targeting injury and violence prevention programs under CFDA 93.136. The Centers for Disease Control and Prevention (CDC), through the Department of Health and Human Services, is soliciting applications (RFA-DD-18-000) with a deadline of September 30, 2030. Eligible applicants include state governments, local governments, territorial governments, county governments, cities, townships, special districts, state-controlled higher education institutions, and tribal governments, with only one application funded per eligible state or territory. Applicants must have access to state or territory-wide data sources and demonstrate organizational capacity in injury and violence prevention, including child abuse prevention, sexual violence prevention, firearm-related violence reduction, and overdose prevention.
Awards will be provided as 12-month initial budget periods with potential performance periods up to five years, subject to fund availability and satisfactory recipient progress. Historical award amounts range from $150,000 to $7.5 million annually, with typical cooperative agreements between $350,000 and $1.4 million. The application review process consists of two phases: Phase I screening for eligibility and completeness, followed by Phase II panel evaluation assessing approach, evaluation and performance measurement capabilities, applicant organizational capacity, scientific merit, personnel qualifications, feasibility, alignment with national health priorities, and budget reasonableness. Non-competitive continuation awards are available after initial funding based on satisfactory performance. Recipients must maintain records for three years beyond project completion and comply with 2 CFR 200 requirements.
