PERFORMANCE WORK STATEMENT (PWS)
USS FRANK CABLE AND USS EMORY S LAND
SUBMARINE EQUIPMENTSUPPORT SERVICES
1.0 BACKGROUND.
1.1 USS FRANK CABLE (AS 40) or USS EMORY S LAND (AS 39) will be assigned as the Lead Maintenance Activity (LMA).
1.2 LMA provides submarine maintenance support equipment to homeported and deployed submarines.
2.0 OBJECTIVE/SCOPE.
2.1 LMA seeks a Contractor to provide all necessary labor, material, transportation, supervision, equipment, engineering design, preventative maintenance and corrective maintenance required to maintain Government furnished submarine maintenance support equipment in a ready for issue condition with delivery, setup and training upon request by the LMA repair department via The Technical Expert, Ship Superintendent, or Production Management Assistant.
3.0 PERFORMANCE REQUIREMENTS.
3.1 Contractor shall obtain from the previous contractor, if applicable, all Government furnished submarine maintenance support equipment, associated hardware and deliver the material to a site of his choosing. Contractor is responsible for all costs relating to this movement and transfer of material and shall handle and transport the material in a safe and responsible manner. The contractor shall provide a written condition report and quote for the necessary parts, repairs, and labor required in support of repairing each piece of equipment back to full operating condition within 14 calendar days of execution of this agreement. The Government will review the condition report and quote and approve funding as appropriate, or determine that LMA personnel will conduct the repairs organically.
3.2 STORAGE
3.2.1 Contractor shall store all equipment listed in the Table of equipment (paragraph 3.2.2), inside a clean, dry, secure area out of the elements. Storage area shall be of permanent structure (not CONEX box, tent, container or similar) and in/on a secure location and remain locked when not occupied.

3.3 DELIVERY
3.3.1 Contractor shall provide submarine service maintenance support equipment delivery including pick up to/from pier side COMNAVMAR or Polaris Point sites.
3.3.2 Contractor will use turnover checklists contained in Attachments (2) and (3) to assist in receipt/turnover documentation for all equipment and components transferred.
3.3.3 Tended unit representative, Contractor and shop 25D representatives will sign for acceptance of equipment after operational test is performed. Contractor will use DD FORM 1149 to receive equipment from tended unit back to the Contractor.
3.3.4 Tended unit representative, Contractor or shop 25D representative will sign return checklist upon successful transfer with accountability of all equipment and material.
3.3.5 Contractor shall provide in writing an equipment condition report of any discrepancies noted when equipment is transferred back to the Contractor. The contractor shall provide the written condition report and quote for necessary parts, repairs, and labor required returning each piece of equipment back to full operating condition by COB on the next working day of following week.
3.3.6 Delivery lead time. Contractor shall deliver equipment within 24 hours of notification Monday through Sunday. Notifications will be made by the LMA’s PMA, Ship Superintendent (SHIPSUP) or designated representative. Deliveries requested which do not meet the 24 hour notification lead time shall be billed as 1.5 deliveries with the exception of broken onsite equipment.
3.3.7 Contractor shall include support hoses, cabling and all necessary hardware for use and shall be kitted in boxes (where applicable) certified Ready For Issue (RFI) with plastic tamper seals affixed by the contractor that will removed by the end user.
3.3.8 All hose ends and end fittings shall be covered with appropriate foreign material exclusion (FME) devices at all times when stowed. Tended unit Representative shall sign for inventory of gear (piece by piece to be verified by serial number, if applicable) when signing for acceptance.
3.3.9 When Contractor picks up equipment, an inventory shall be performed and any missing parts annotated in writing by serial number. Tended unit representative and the Contractor shall sign for acceptance of equipment upon delivery and pick up.
3.3.10 Contractor shall be responsible for truck transportation of equipment and any electrical extensions necessary to include any plugs, cables and hardware.
3.3.11 With the exception of chlorinators and de-chlorinators, 25D personnel shall provide training, paper operating procedures, and logs for each type of equipment. Tended unit shall sign for receipt of the training and procedures and shall be responsible for proper operation, monitoring and cleaning of equipment while it is in their custody. Commander, Submarine Squadron Fifteen N4 will provide chlorinator/de-chlorinator training and logs for proper operation.
3.3.12 Contractor shall provide a logbook to the tended unit for logging data for HPACs, LPACs, Diesel powered fire pumps, and hydrolancers.
3.3.13 Contractor shall deliver a berm with all HPAC systems. Berm will be of sufficient size to enclose the entire HPAC frame and will be of sufficient volume to contain the entire oil content of the HPAC if a leak should occur. Contractor will also provide a berm for the HPAC overflow bottle of sufficient volume to contain the entire volume of the bottle if the bottle overflows.
3.3.14 Contractor shall not connect equipment directly into Tended Unit’s system. Equipment will be connected mechanically and electrically by Tended Unit’s personnel.
3.3.15 Contractor shall not connect equipment to pier side shore power bunkers and will coordinate with BOSC/DZSP for connecting/disconnecting.
3.3.16 Equipment will be delivered in a rotating manner in order to equalize runtime between units of the same type.
3.3.17 Complete inventory of equipment, components, component locations, and repair parts shall be submitted by Close of Business (COB) on the first day of each work week to include repair parts and status of ordered parts including their Estimated Delivery Dates (EDDs).

Note 1: Oil Pickup and deliveries will be up to 3 pallets of 2190TEP oil per deliveries.
Note 2: Man-hours for filtration will only be used for filtering 2190 TEP lubricating oil not for trouble shooting or repair of the equipment.
Note 3: Man-hours deduction report will be reviewed and approved by 25D Representative and/or Tender Unit Representative.
3.6 OPERATIONS
3.6.1 If any submarine service equipment fails, Contractor shall provide onsite support and any necessary corrective maintenance to bring equipment immediately back to operation. If equipment cannot be re-started, contractor shall replace with a RFI unit within 4 hours followed up with a man-hour deduction report after onsite support have been taken. A condition report shall be submitted to bring the replaced equipment back to full operating condition.
3.6.2 Contractor shall replenish HPAC oil if level falls below technical manual specified level or as reported by Tended Unit. Contractor shall conduct daily checks of oil levels.
3.6.3 Contractor shall be responsible for checking HPAC overflow bottles daily, including weekends and federal holidays. When bottles are greater than 50% full, Contractor is responsible for ensuring that the HAZMAT is disposed of and shall be responsible for overflow cleanup/EPA consequences if a spill occurs due to negligence.
3.6.4 Contractor shall conduct daily checks of fuel levels. Contractor shall provide and refuel all equipment with 24 hours of discovery, and ensure that no run time is lost due to lack of fuel.
**NOTE** Submarine force will be responsible for the oversight while the oil is being filtered at all times and will also take sample and deliver samples to PMT.
3.6.5 Contractor will transport 2190 TEP oil and will filter the oil inside their facilities with the ships force oversight, ships force will take the initial sample and after 3 passes through the filter and deliver to PMT. If the initial sample result is over 500000 ppm the oil will be deemed unsat for filtering and return the oil to supply. If the oil is below 500000 ppm contractor will continue to filter and replace filter as needed until sample result is sat. Once oil is sat contractor will deliver oil with oil rig and berm.
3.7 MAINTENANCE
**NOTE** Preventative maintenance shall be performed at Contractor’s facility.
3.8 DEFINITIONS
3.8.1 Clean: Equipment shall be free from dirt and debris and in the case of chlorinators/dechlorinators a fresh water flush will have been performed since last use in accordance with appropriate technical documents. All equipment will be stored clean, dry and in Ready for Issue (RFI) condition. The contractor shall have a designated allowance fund of $1500 to purchase routine consumables ie. oil filters, oil belts, coolant, fittings or other necessary expenditures to maintain equipment.
3.8.2 Ready for Issue (RFI): Equipment will be operational with no outstanding maintenance to be accomplished before use to include support hoses, cabling and all hardware necessary for use where applicable extraneous material shall be kitted in boxes certified ready for use with plastic tamper seals affixed by the contractor to be removed by the end user. Equipment shall be kept in good, issuable condition, free of visible corrosion and wear. Worn hoses with damaged outer coating will not be issued. At no time will a hose with exposed braiding be issued.
3.8.3 Not Ready For Issue (NRFI): Equipment determined to be in Non-RFI condition due to preventative maintenance, needing repair or periodic inspection shall be annotated on the Weekly status report and expeditiously brought back to a RFI condition. If the equipment is not returned to RFI condition within 1 week from notification of NRFI condition the Contractor shall notify the Technical POC in writing with reason for the delay and a plan to bring the equipment to RFI condition with an estimated date.
3.9 PREVENTIVE MAINTENANCE
3.9.1 Preventative maintenance shall be per comprehensive maintenance plan.
a. Chlorinators:
(1) Must be flushed with fresh water after each use.
(2) Prior to issue and upon receipt back from a unit, Contractor will inspect electronics enclosure assembly/pump face and cover for cleanliness and absence of moisture.
**Note** An inspection and cleaning of the chlorine generation cells is necessary for reliable operation of the chlorinators.
(3) Contractors will provide a binder attach to equipment keeping track of all maintenance completed and hours of usage, and will be signed and dated by Tended Unit.
b. Dechlorinators:
(1) Must be flushed with fresh water after each use.
(2) Prior to issue and upon receipt back from a unit, Contractor will inspect electronics enclosure assembly/pump face and cover for cleanliness and absence of moisture.
(3) Contractors will provide a binder attach to equipment keeping track of all maintenance completed and hours of usage, and will be signed and dated by Tended Unit.
c. High Pressure Air Compressors (HPAC)
(1) Contractor shall pressure wash with fresh water after each use.
(2) Contractor shall perform test of safety relief valve at least quarterly. If a HPAC has not been placed in service during the previous three months, the relief valve test can be performed prior to delivery vice quarterly.
(3) Contractor shall check proper oil level as per technical manual prior to delivery.
(4) Contractor will ensure that electrical cables to the HPACs are securely mounted in an OEM specification stuffing box on the controller. Electrical tape or putty will not be substituted for the stuffing box or used in conjunction with the stuffing box.
(5) Contractor will preserve compressor quarterly to remove surface rust. Preservation will include painting cleaned surfaces on the compressor unit and the frame.
(6) Contractor will repack the trailer mounted HPAC wheel bearings annually.
(7) Contractors will provide a binder attach to equipment keeping track of all maintenance completed and hours of usage, and will be signed and dated by Tended Unit.
d. Hydrolancers
(1) Crankcase oil will be sampled after each use or quarterly (whichever comes first) for evidence of sediment or presence of water. If water or sediment is visible, oil must be drained, flushed and replaced.
(2) Contractor will ensure that electrical cables to the hydrolance rigs are securely mounted in an OEM specification stuffing box on the controller. Electrical tape or putty will not be substituted for the stuffing box or used in conjunction with the stuffing box.
(3) Contractor shall pressure wash with fresh water after each use.
(4) Contractor will preserve hydrolance rigs quarterly to remove surface rust. Preservation will include painting of angle iron frames of hydrolance rig, if installed.
(5) Prior to issue to a tended unit, Contractor will check lube oil for presence of water.
(6) Upon return from a tended unit, Contractor will check oil and relief valve to ensure rupture disk did not rupture causing water to leak past the valve.
(7) Contractor shall clean air filter on diesel powered model at least once every three months. If the hydrolancer has not been placed in service during the previous three months, the air filter cleaning can be performed prior to delivery vice quarterly.
(8) Contractor shall change engine oil and replace oil filter on diesel powered model at least once every three months. If the hydrolancer has not been placed in service during the previous three months, the oil and filter change can be performed prior to delivery vice quarterly. However, the oil and filter will be changed at least annually, even if the hydrolancer is not issued for use.
(9) Contractors will provide a binder attach to equipment keeping track of all maintenance completed and hours of usage, and will be signed and dated by Tended Unit.
e. Low Pressure Air Compressors (LPAC)
(1) Contractor shall perform a check of battery to ensure proper operation at least once every three months.
(2) Contractor shall test properties of antifreeze using antifreeze test strips at least annually.
(3) Contractor shall clean air filter at least once every three months. If a LPAC has not been placed in service during the previous three months; the air filter cleaning can be performed prior to delivery vice quarterly.
(4) Contractor shall change engine oil and replace oil filter at least once every three months. If a LPAC has not been placed in service during the previous three months, the oil and filter change can be performed prior to delivery vice quarterly. However, the oil and filter will be changed at least annually, even if the LPAC is not issued for use.
(5) Contractor shall change compressor oil and replace compressor oil filter at least once every three months. If a LPAC has not been placed in service during the previous three months, the oil change can be performed prior to delivery vice quarterly. However, the oil will be changed at least annually, even if the LPAC is not issued for use.
(6) Contractor shall replace fuel filter once per year.
(7) Contractor shall replace compressor separator element once per year.
(8) Prior to issue to a tended unit, Contractor will check oil and fuel for presence of water.
(9) Contractors will provide a binder attach to equipment keeping track of all maintenance completed and hours of usage, and will be signed and dated by Tended Unit.
f. Diesel Powered Light Towers
(1) Contractor shall perform a check of battery to ensure proper operation at least once every three months.
(2) Contractor shall test properties of antifreeze using antifreeze test strips at least annually.
(3) Contractor shall clean air filter at least once every three months. If a light tower has not been placed in service during the previous three months, the air filter cleaning can be performed prior to delivery vice quarterly.
(4) Contractor shall change engine oil and replace oil filter at least once every three months. If a light tower has not been placed in service during the previous three months, the oil and filter change can be performed prior to delivery vice quarterly. However, the oil and filter will be changed at least annually, even if the light tower is not issued for use.
(5) Contractor shall lubricate the tower and cabling IAW technical manual at least once every 6 months.
(6) Contractor shall repack wheel bearings at least annually.
(7) Prior to issue to a tended unit, Contractor shall check oil and fuel for presence of water.
(8) Contractors shall provide a binder attach to equipment keeping track of all maintenance completed and hours of usage, and will be signed and dated by Tended Unit.
g. Diesel Powered Fire Pump
(1) Contractor shall perform a check of battery to ensure proper operation at least once every three months.
(2) Contractor shall test properties of antifreeze using antifreeze test strips at least annually.
(3) Contractor shall clean air filter at least once every three months. If a fire pump has not been placed in service during the previous three months, the air filter cleaning can be performed prior to delivery vice quarterly.

If a fire pump has not been placed in service during the previous three months, the oil and filter change can be performed prior to delivery vice quarterly. However, the oil and filter will be changed at least annually, even if the fire pump is not issued for use.
(5) Prior to issue to a tended unit, Contractor will check oil and fuel for presence of water.
(6) Contractors shall provide a binder attach to equipment keeping track of all maintenance completed and hours of usage, and will be signed and dated by Tended Unit.
h. Electrically Powered Fire Pump
(1) Contractor shall ensure that electrical cables to the fire pump are securely mounted in an OEM specification stuffing box on the controller. Electrical tape or putty will not be substituted for the stuffing box or used in conjunction with the stuffing box.
(2) Contractor shall maintain one spare set of drive belts for the unit.
3.9.2 Contractor shall conduct operational checks and make the submarine support equipment RFI immediately upon pick up of equipment and prior to returning equipment to storage facility.
3.9.3 Contractor shall ensure that a minimum of 4 RFI High Pressure Air Compressors, 6 RFI Chlorinators, 6 RFI Dechlorinators and 3 RFI Hydrolancers (4 RFI Hydrolancers), 3 RFI Light Towers, and 7 Low Pressure Air Compressors are able to be delivered at all times (equipment in use by tended units counts toward this total). Required hoses and fittings to support the minimum number of simultaneous deliveries outlined above are also required to be maintained on hand in an RFI status.
i. Industrial Dehumidifiers
| 3.9.4 | Contractor shall ensure that maintenance on industrial humidifiers are performed in accordance with OEM provided by Work Center 25D. |
| 3.10 | CORRECTIVE MAINTENANCE |
3.10.1 With the exception of paragraph 3.3.4, Contractor shall provide in writing a written condition report of any discrepancies noted. The written condition report and quote for necessary parts, repairs, and labor required to return each piece of equipment back to full operating condition shall be provided within 2 days of discovery of discrepancy.
3.10.2 If LMA elects to perform repairs organically, the contractor shall provide technical support; including troubleshooting support, technical data, and parts information on equipment components and assemblies. Technical support shall be available on a near-real-time basis via telephone. All requests for technical support shall be answered by the contractor within one business day in the same manner it was submitted. Any unresolved activity shall be updated on a daily basis until resolution is satisfactorily completed.
3.11 TRAINING
3.11.1 Training of contractor personnel in the repair and operation of temporary services equipment, will be arranged by the contractor, and shall be at the contractor’s expense.
3.11.2 With the exception of chlorinators and de-chlorinators, Shop 25D shall provide training to supported units, outlining proper operation of the equipment.
3.12 DELIVERABLES
3.12.1 Weekly Status Report. Contractor shall provide a weekly report, via email, with an up to date status report of all service equipment on the first business day of the week. Template of weekly status report is provided as attachment (4). The report shall contain equipment name, serial number, location, status of equipment, any equipment off line for major service or corrective maintenance and expected return to service date. The report shall include all deliveries or pick-ups that occurred in the reporting week, current diesel fuel consumption and refueling times. Inconsistencies in this report shall require the contactor to resubmit the report within 24 hours. The report shall be submitted to the Technical POC every Tuesday by 0900.
3.12.2 Contractor will, within 14 calendar days of execution of this agreement, develop and provide to the Technical POC a comprehensive preventative maintenance plan to capture these requirements delineated in paragraph 3.4.2. The plan shall be briefed each month to Shop 25D representatives, and a status update of maintenance performed and planned shall be forwarded to the Technical POC on the 5th day of each month. The plan shall demonstrate maintenance completed and maintenance plan for the upcoming month.
3.12.3 Contractor shall issue a report of missing components parts and subassemblies to the Technical POC for material history within 2 business days of the discovery of the missing items.
3.12.4 Contractor shall allow USS FRANK CABLE and USS EMORY S LAND to inspect facility at any time without notice.
3.12.5 The contractor shall be responsible to acquire and maintain a library of all technical manuals required to service and equipment. All appropriate technical manuals on hand at the time the work is being performed.
3.12.6 The contractor shall provide copies of applicable technical manuals, parts manuals and diagnostic manuals for equipment via CD and paper copy within 60 days of execution of this agreement.
3.12.7 The contractor shall have available phone number for 25D representative to contact at any given time for emergency requirements.
3.13 SCHEDULE DELIVERY SUMMARY TABLE B

4.0 GOVERNMENT FURNISHED ITEMS.
4.1 GENERAL. The Government may from time to time replace or provide additional services, material, or Government furnished equipment as part of this PWS. In the event new equipment needs to be added to this PWS, the contract will be subject to a modification. The Contractor will be notified 14 days in advance and allowed the opportunity to determine the cost associated with the addition. The new equipment will be added to the PWS if the cost is determined fair and reasonable by the Contracting Officer.
4.2 INVENTORY. A joint operational equipment check and inventory shall be conducted and signed for acceptance by the LMA Repair Department representatives and the Contractor upon award of the contract. Upon receipt of equipment the Contractor has 30 days to bring all equipment to RFI condition.
5.0 GOVERNMENT COORDINATED SUPPORT SERVICES.
5.1 Pier side crane and MHE support services for handling FRANK CABLE owned equipment listed in paragraph 3.2.2 Table A.
5.2 Pier side electrical connection, for electrically powered equipment (i.e.
HPAC, hydrolance, and fire pump).
5.3 Request for this support via DZSP21 and NAVFAC shall be arranged and authorized by the Technical POC.
6.0 QUALITY ASSURANCE.
6.1 The Contractor is responsible for implementing and maintaining a Quality Control Plan (QCP) that identifies potential and actual problems that results in the correction of those potential and actual problems. The QCP shall be implemented on the first day of contract performance and a copy of QCP shall be provided to the Technical POC.
6.1.1 The QCP shall outline the Contractor’s plan to ensure the quality of deliverables as outlined in this PWS. The QCP shall address what will be reviewed and shall outline actions to be taken by the Contractor, should the review(s) identify areas requiring remedial action.
6.1.2 The Contractor shall keep records of all inspections and corrective actions taken, as outlined in the QCP and shall make such records available to the Technical POC upon request.
6.2 Periodic Progress Meetings. The Technical POC and/or other Government personnel, as appropriate, may meet with the Contractor to review the Contractor's performance. At this meeting(s) the Technical POC will apprise the Contractor of how the Government views the Contractor's performance and the Contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government.
7.0 CONTRACTOR QUALIFICATIONS/REQUIREMENTS.
7.1 Per current Department of Defense and Homeland Security requirements, performance of all work under this contract is restricted to US citizens and US nationals only. The Contractor shall provide proof of this requirement upon request of the Technical POC.
7.2 Contractor Employees and Vehicles at all times must be able to gain general access to Naval Base Guam and Polaris Point Piers.
8.0 PERIOD OF PERFORMANCE: 01 July 2020 – 30 June 2021.
8.1 WORKING HOURS. Regular working hours shall consist of an 8-hour period, between 8:00 AM - 4:00 PM Monday through Sunday, including Federal holidays. However, contractor shall deliver or repair/replace malfunctioning equipment within 4 hours of notification Monday through Sunday, regardless of business hours. Notifications will be made by the LMA PMA, SHIPSUP or Shop 25D representative.
8.2 Contractor shall be responsible for obtaining this equipment from the present storage location in Agat, Guam.
