About this file

The document is a Past Performance Questionnaire for a valet and parking attendant contract (Solicitation Number 36C24225Q1036) at the Northport VA Medical Center. The evaluation form is designed to capture detailed performance metrics across six key areas: quality of service, recruitment/retention, timeliness/schedule, business relationships, customer satisfaction, and overall contract performance. The form requires a comprehensive rating using an adjectival scale from Outstanding (O) to Unacceptable (U), with specific emphasis on staffing, equipment provision, communication, and veteran service quality.

The questionnaire must be completed and returned directly to Dolores.Sachuk-Thompson@va.gov by September 18, 2025, at 4:30 PM ET, with special instructions that any Outstanding ratings require supporting narrative explaining the vendor's benefits. The document is part of a solicitation by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 2, seeking evaluation of a contractor's performance for valet and parking lot attendant services. Additional sections probe whether the contractor would be recommended for future contracts, if there were any terminations or performance issues, and require a signature from the evaluating agency/company representative.
