About this file

This document is a Past Performance Questionnaire used by the Department of Veterans Affairs to evaluate an offeror's performance on Outdoor Event Tent Rental Services. The questionnaire is provided to third-party references (companies that have previously contracted with the offeror) to assess the offeror's capability and reliability. All information provided is classified as Evaluation Information, protected from disclosure under the Freedom of Information Act, with reference sources remaining confidential.

The questionnaire requires assessors to rate the offeror across five performance categories using a four-tier rating scale: Exceptional, Satisfactory, Unsatisfactory, or Not Applicable. The performance areas evaluated are: (1) Delivery and Scope—whether agreed work was completed; (2) Quality—whether work met agreed standards; (3) Timeliness—whether deadlines were met; (4) Financial Performance—whether prices, invoices, and payments were consistent with the agreement; and (5) Compliance and Management—whether other obligations were met and issues managed effectively. Assessors are encouraged to provide written comments for all ratings and must provide narrative explanations specifically for ratings of Exceptional or Unsatisfactory. The completed questionnaire requires the assessor's company name, name, title, email address, and date of submission.
