About this file

This is a Performance Assessment Questionnaire used by the U.S. Army to evaluate contractor performance on federal contracts. The questionnaire must be completed and returned to Contracting Specialist Ms. Sonya Van Valkenburg at Sonya.d.vanvalkenburg.civ@army.mil no later than April 7, 2026 by 11 a.m. Eastern Time.

The form consists of three parts. Part I requires the contractor or offeror to provide contract identification information including company name, address, program title, contract number, contract type, prime contractor name (if applicable), contract award date, completion date, nature of contractual effort, total contract value, and identification of the offeror's representative. Part II requests evaluation responses from a point of contact or respondent across five performance criteria: compliance of products, services, documents, and deliverables with specification requirements and workmanship standards; effectiveness of project management including subcontractor use and control; timeliness of performance for services and product deliverables; effectiveness in forecasting and controlling estimated costs (for cost reimbursement contracts only); and commitment to customer satisfaction and business-like concern for customers' interests. Each evaluation criterion uses a four-tier rating scale: Substantial Confidence, Satisfactory Confidence, Limited Confidence, or No Confidence, with mandatory rationale required for all responses. Part II also includes sections for general comments, identification of other relevant past efforts and associated points of contact, and respondent identification. Part III provides return instructions directing completion of the questionnaire to the identified contract specialist.

Work was accomplished with few, if any, minor problems for which corrective actions taken by the contractor were highly effective. The offeror has been highly successfully in performing the required effort.

Satisfactory Confidence: Performance meets contractual requirements and exceeds some requirements that benefit the end user. Work was accomplished with some minor problems for which corrective actions taken by the contractor were effective. The offeror has successfully performed the required effort.

Limited Confidence: Performance does not meet some contractual requirements.

Serious problems with contractor performance were experienced for which the contractor has either not yet identified corrective actions or the corrective actions taken appear only marginally effective. The offeror has had little success performing the required effort.
