About this file

This is a Past Performance Questionnaire template used by the U.S. Government Publishing Office Acquisition Services to evaluate contractor performance on completed federal contracts.

The form is divided into two sections: contractor-supplied information and evaluating organization information. The contractor section requires completion of basic contract details including contractor name and address, project name, contract number, contract date, period of performance, pace of performance, completion date, contract value, and contract type. Contractors must also provide a description of the project's key characteristics. The evaluating organization section captures the name and address of the evaluating organization, the evaluator's name, position during project execution (Project Manager, Contracting Officer's Representative, or other), phone number, and the number of months the evaluator monitored performance. The questionnaire includes a rating matrix covering nine performance areas: Quality of Products and Services, Overall Performance, Schedule, Technical Requirements, Cost Control, Subcontracting, Customer Satisfaction in routine matters, Customer Satisfaction in unexpected difficulties, and Repeat Business. Each area uses a six-point scale (EX=Exceptional, VG=Very Good, S=Satisfactory, MG=Marginal, US=Unsatisfactory, N/O=Not Observed). The form also includes fields for listing contractor weak points and strong points, other completed projects, and average award fee percentage if applicable. Completed questionnaires are classified as Source Selection Sensitive Information under 41 U.S.C. 423.
