About this file
This is a Past Performance Questionnaire template for federal contract proposals submitted to the U.S. Department of Labor, Bureau of Labor Statistics (BLS). The questionnaire is designed to be completed by references or previous clients to evaluate contractor performance on prior contracts relevant to the solicitation 1605C5-26-Q-00008. Responses must be submitted to Contract Specialist Daniel Garza at garza.daniel@dol.gov no later than 12:00 PM Eastern Daylight Time on May 22, 2026, and must be sent from a corporate email account. An alternate point of contact should be identified if the primary contact will be unavailable between May 22–26, 2026 for verification purposes.
The questionnaire evaluates contractor performance across five primary areas: (1) ability to meet requirements, (2) timeliness of product/service delivery and ability to resolve timeliness issues, (3) quality of content/shipments, (4) overall contract management, and (5) overall contractor performance regarding requirements, schedule, and price. Respondents rate performance using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory) with an N/A option, and are requested to provide explanatory narratives for each response. The questionnaire also collects information about the referenced contract, including contract number, value, period of performance, description of items/services provided, and contact information for the person completing the evaluation. This is a template document and does not address specific products, services, or award details for the current solicitation.
