About this file

The document is a Past Performance Questionnaire issued by the Federal Bureau of Investigation (FBI) for the Quantico Lead Cleaning Services contract. The questionnaire is designed to evaluate potential contractors' past performance across six key criteria: quality of service, schedule, cost control, business relations, management of key personnel, and utilization of small business. The rating scale ranges from Exceptional to Unsatisfactory, with references asked to provide detailed assessments and comments for each performance area.

The questionnaire is specifically related to the FBI Quantico Complex Lead Cleaning Services, which covers lead cleaning services for gun firing ranges, classrooms, physical training areas, bathrooms, and other training support areas across the 547-acre complex with 130+ buildings. The document instructs past performance references to complete the form and email it directly to Contracting Officer Maureen Murdock and Contract Specialist Aaron Awai, with the subject line "{FBI- Quantico Lead Cleaning/PPQ}" followed by the company's name. The final section of the questionnaire also requests an overall assessment of the contractor's future potential and any reservations about soliciting their services.
