About this file
This is a Past Performance Questionnaire template designed to collect and evaluate contractor performance information for federal contract evaluation purposes.
The form is divided into two primary sections: contractor-completed blocks and client-completed blocks. Contractors must provide firm identification (name, CAGE code, address, DUNS number, contact information), work performance role (prime contractor, subcontractor, joint venture, or other), contract details (number, type, title, location, award and completion dates, original and final pricing), and a project description including complexity level and relevance to the current solicitation. Clients must complete identification information, describe their role in the project, and date and sign the questionnaire. The performance evaluation section uses a seven-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, Not Available) across seven major categories: Quality (technical data preparation, quality standards, problem resolution, quality control programs); Schedule/Timeliness (delivery compliance, resource utilization); Customer Satisfaction (end user satisfaction, contractor cooperation, overall satisfaction); Management/Personnel/Labor (on-site management, workforce qualifications, government property control, personnel expertise, small business utilization, multi-project management, requirement changes, overall management); Cost/Financial Management (price compliance, cost reduction innovations, invoice timeliness for cost-type contracts, accounting system adequacy, contract terminations, financial stability); Safety/Security (safety program adherence, security compliance); and General (emergency response, contractual compliance, future work willingness, overall performance rating). The template requires narrative explanations for any unsatisfactory or marginal ratings and concludes with open-ended sections for documenting contractor strengths, weaknesses, deficiencies, and additional comments relevant to performance risk assessment.
